Nhiễm cytomegalovirus (CMV) là một nguyên nhân quan trọng gây bệnh thần kinh trong bối cảnh nhiễm virus suy giảm miễn dịch ở người (HIV) giai đoạn muối và được công nhận là một nguyên nhân của hội chứng viêm phục hồi miễn dịch (IRIS) sau khi bắt đầu liệu pháp kháng retrovirus hoạt tính cao (HAART). Viêm mạch hệ thần kinh trung ương thứ phát do CMV chỉ hiếm khi được mô tả trong bối cảnh HIV, mặc dù khả năng gây nhiễm cho các tế bào nội mô vi mạch trong não của CMV đã được thiết lập. Tuy nhiên, chúng tôi báo cáo một trường hợp thể hiện mối liên hệ giữa CMV và nhiều tổn thương nhồi máu não mạch nhỏ xảy ra sau khi bắt đầu HAART.
