Hội chứng viêm vú dạng hạt, ban đỏ nút và viêm khớp: tổng quan dựa trên các trường hợp

Viêm vú dạng hạt (GM) là một dạng hiếm gặp của tình trạng viêm vú, liên quan đến đau ngực đơn phương hoặc song phương, sưng tấy và sự hình thành khối u. Mặc dù vẫn chưa hiểu rõ được cơ chế bệnh sinh, một số báo cáo đã liên kết GM với các biểu hiện như ban đỏ nút và thỉnh thoảng với viêm khớp, cho thấy GM có thể có một thành phần bệnh tự miễn. Chúng tôi nhằm mô tả hai trường hợp tồn tại song song của GM, ban đỏ nút và viêm khớp. Chúng tôi cũng đã thực hiện một cuộc tổng quan tài liệu để đánh giá toàn diện và mô tả đặc điểm của các bệnh nhân mắc GM, ban đỏ nút và viêm khớp, cũng như xác định các lựa chọn điều trị hiệu quả. Một cuộc tổng quan tài liệu đã được tiến hành sử dụng PubMed và EMBASE, và thu được 14 báo cáo/trường hợp, với tổng số 29 bệnh nhân. Tất cả bệnh nhân đều là nữ và tổn thương vú đơn phương xuất hiện rõ ràng ở đa số bệnh nhân. Chín bệnh nhân (31%) có triệu chứng viêm khớp, 6 bệnh nhân (20.7%) bị sốt, và 6 bệnh nhân (20.7%) phát triển triệu chứng trong thời kỳ mang thai. Tất cả bệnh nhân đều có phim chụp ngực bình thường và cấy mô đều âm tính. Trong hầu hết các trường hợp (n=25, 86.2%), sự cải thiện triệu chứng được ghi nhận với glucocorticoid và bốn bệnh nhân (13.8%) đã trải qua phẫu thuật điều trị cho GM. Với các đặc điểm lâm sàng của bệnh nhân mắc GM, có ban đỏ nút, có hoặc không có viêm khớp, và phản ứng tích cực với glucocorticoid, chúng tôi đề xuất rằng kiểu hình được mô tả đại diện cho một bệnh tự miễn hệ thống chưa được công nhận, có thể được gọi tắt bằng cụm từ “GMENA” (hội chứng viêm vú dạng hạt, ban đỏ nút, viêm khớp). Cần có thêm các nghiên cứu để làm sáng tỏ cơ chế bệnh sinh của hội chứng này.
