A 48-year-old male patient was admitted to our hospital with dyspnea accompanied by orthopnea. Chest x-rays showed a cardiothoracic ratio of 68% and pulmonary congestion. He was diagnosed with dilated cardiomyopathy. β-Blocker (Carvedilol) therapy was initiated on Day 22 of the disease using a small initial dose. He was followed up based on BNP levels and MIBG scintigraphy. The H/M ratio and MIBG washout rate were 1.98 and 33.4%, respectively, on. Day 20 and 2.15 and 28.1%, respectively, on Day 72. The patient was discharged on Day 72 when congestive heart failure improved. Relatively high BNP levels were observed for 1 month after starting treatment with a β-blocker. Plasma BNP levels were still as high when his heart failure was improved. BNP is useful as a convenient indicator for the severity of cardiac diseases. MIBG scintigraphy may be used thereafter to evaluate the severity in greater detail and more precisely determine the prognosis.

Our aim was to evaluate regional differences between brain activity in two resting control conditions measured by 3D PET after administration of FDG through either the intravenous (i.v.) or the oral route. Ten healthy male volunteers engaged in the study as the i.v. group (mean age, 26 ±9.3 years, ±S.D.) who received FDG intravenously and another 10 volunteers as the oral group (mean age, 27.9 ±11.3 years, ±S.D.) who received FDG per os. A set of 3D-PET scans (emission and transmission scans) were performed in both groups. To explore possible functional differences between the brains of the two groups, the SPM-96 software was used for statistical analysis. The results revealed that glucose metabolism was significantly higher in the superior frontal gyrus, superior parietal lobule, lingual gyrus and left cerebellar hemisphere in the i.v. group than in the oral group. Metabolically active areas were found in the superior, middle and inferior temporal gyrus, parahippocampal gyrus, amygdaloid nucleus, pons and cerebellum in the oral group when compared with the i.v. group. These differences were presumably induced by differences between FDG kinetics and/or time-weighted behavioral effects in the two studies. This study suggests the need for extreme caution when selecting a pooled control population for designated activation studies.
Evaluation of the effects of chemotherapy on brain glucose metabolism in children with Hodgkin’s lymphoma

Chemobrain is a recently proposed pathological entity. 18F-FDG PET/CT can show objective abnormalities to explain brain disorders caused by chemotherapy, although no study has investigated these phenomena in children to date. The main objective of the present study was to examine quantitatively the effects of chemotherapy on brain metabolism in a homogeneous population of children treated for Hodgkin's lymphoma using 18F-FDG PET/CT. In this retrospective study, we included 20 children, newly diagnosed with Hodgkin’s lymphoma, who underwent 18F-FDG PET/CT at initial staging and at least one PET/CT in follow-up. The SPM12 software provided t-maps to show the difference in metabolism between these PET/CTs. The statistical maps were analyzed with xjView software to identify the brain regions associated with the clusters detected. Altered glucose metabolism was found in the frontal, cingular, and temporoinsular regions after two cycles of chemotherapy. Results in children were compared to a group of 35 adults. For the same statistical threshold, the extent and depth of the metabolic alterations were less in the adult group than in children. 18F-FDG PET/CT is useful in providing objective data to explain brain disorders caused by chemotherapy. This could lead to better care and should be compared to neuropsychological test results.

Usefulness of FDG-microPET for early evaluation of therapeutic effects on VX2 rabbit carcinoma

The aim of this study was to determine the potential use of high-resolution FDG-microPET for predicting the primary effects of radiotherapy and/or hyperthermia on tumor-bearing rabbits. Twenty-eight VX2 xenografts in the thighs of rabbits were divided into the following 5 treatment groups: radiotherapy at a single dose of 10, 20 or 30 Gy, hyperthermia (43 degrees Celsius, 1 hour), and the combination of radiotherapy and hyperthermia ( 10 Gy + 43 degrees Celsius, 1 hour). FDG-microPET images were obtained by using a microPET P4 system at pretreatment and at 24 hours and 7 days after treatment. For the evaluation by FDG-microPET, tumor/muscle (T/M) ratios, retention index [RI = (T/M ratio at 120 min - T/M ratio at 60 min) / T/M ratio at 60 min], and time activity curve (TAC) were acquired. We divided the xenografts into a responder group (partial response + stable disease, n = 14) and a non-responder group (progressive disease, n = 14). The T/M ratio at 24 hours after the treatment in the responder group was decreased remarkably with that at pre-treatment (p < 0.05), while in the non-responder group it showed no significant change between the time points. The RI and TAC patterns were comparable to T/M ratios in each treatment group. T/M ratios, RI, and TAC indicated marked changes at the time point of 24 hours in the responder group, although the tumors did not show any significant change in volume at that time. Photomicrographs of sections showed that the number of viable tumor cells in the responder group decreased at 24 hours after treatment and that inflammatory cell infiltration was marked and almost all viable tumor cells had disappeared by day 7 after treatment. These results suggest that early evaluation by FDG-microPET, especially 24 hours after treatment, is useful to predict the primary effects of the treatment. Histological analysis showed that inflammatory cell infiltration at 7 days after treatment was considered to be a cause of accumulation of FDG in the tumors that showed a significant decrease in tumor cell number. This false-positive should be noted when predicting tumor response by FDG accumulation.

We encountered a 9-year-old Japanese girl with Alagille syndrome. Her scintigraphic examinations of the liver were performed at the ages of 16 months and 9 years.99mTc-PMT, a hepatobiliary imaging agent, was distributed homogeneously in the liver at the younger age, but unevenly produced an area of focally increased uptake in the medial segment of the liver surrounded by peripheral atrophy at the older age.99mTc-GSA, a hepatoreceptor binding agent, was highly accumulated in the area, corresponding to the focally increased uptake of99mTc-PMT. These imaging findings suggest that the pathophysiological and morphological changes of the liver occurred in our patient during the clinical course.
F-18 FDG PET/CT imaging of low-grade mucoepidermoid carcinoma of the bronchus

Mucoepidermoid carcinomas in the bronchial tree are extremely rare tumors. Such tumors are classified into low-grade and high-grade on the basis of histological criteria. Fluorine-18-fluorodeoxyglucose positron emission tomography (F-18 FDG PET) is a useful technique for the evaluation of pulmonary lesions; however, to our knowledge, F-18 FDG PET findings in mucoepidermoid carcinoma of the bronchus have been described in only a few cases. Identifiable focal F-18 FDG uptake has been reported in high-grade mucoepidermoid carcinoma, but it is unclear whether F-18 FDG accumulates in low-grade mucoepidermoid carcinoma. Here, we present the case of a 37-year-old woman, with pathologically proven low-grade mucoepidermoid carcinoma, who underwent high-resolution computed tomography (CT) and F-18 FDG PET/CT before treatment.
