Paronychia with Pyogenic Granuloma in a Child Treated with Indinavir: The Retinoid-Mediated Side Effect Theory Revisited <b>Background:</b> The introduction of HIV-1 protease inhibitors into the treatment of patients infected with HIV-1 has had a major influence on clinical practice. However, the use of protease inhibitors is frequently associated with the development of resistance and several side effects and interactions with other drugs have been reported. <b>Observations:</b> We present the first pediatric patient with paronychia with pyogenic granuloma associated with the administration of the protease inhibitor indinavir. Clinical findings are discussed in view of a possible interference of indinavir with endogenous retinoid metabolism. <b>Conclusion:</b> Considerable evidence advocates the mediation of indinavir side effects by impaired oxidative metabolism of retinoic acid through the inhibition of cytochromes P450 3A by indinavir rather than by impaired formation of 9-cis-retinoic acid.

HIV-Associated Eosinophilic Folliculitis and Follicular Mucinosis The term HIV-associated eosinophilic folliculitis (EF) designates an idiopathic dermatitis that appears in HIV-infected patients with different clinical manifestations but with a distinctive histological feature characterized by a predominantly eosinophilic infiltrate in the follicular infundibula. On the other side, follicular mucinosis (FM) is a reaction pattern in the follicular epithelium, characterized by a mucinous degeneration of the outer sheath of follicles and sebaceous glands. It has been described in association with a variety of unrelated conditions. We report 2 HIV-infected patients with a pruritic papular eruption. Histopathological study revealed the coexistence of EF and FM. To our knowledge, this is the first report that describes this association. The possible relationship between these two entities is discussed.

Lipodystrophia centrifugalis abdominalis juvenilis 8 Korean children exhibited well-defined depressed lesions of the skin accompanied by slight redness of the surrounding area. First these lesions appeared in the inguinal area and then spread peripherally to the navel region and the ipsilateral vulval area. In these 8 cases, 7 occurred in girls and 1 in a boy. The age of onset ranged from 2 to 9 years. Biopsy specimens taken from the well-depressed area showed a decrease in or absence of, subcutaneous fat and a mild inflammatory infiltrate of lymphocytes and histiocytes in the lower dermis and subcutis. These 8 cases are the first, to our knowledge, to be reported outside Japan and considering the age of onset of the dermatosis it is not confined to infants. Therefore, we would like to describe this entity under the title of lipodystrophia centrifugalis abdominalis juvenilis instead of lipodystrophia centrifugalis abdominalis infantilis.

Cutaneous Reactions in Patients with Chronic Hepatitis C Treated with Peginterferon and Ribavirin <b><i>Background/Objective:</i></b> Peginterferon alfa and ribavirin could cause various cutaneous reactions. Treatment-related cutaneous reactions are common, and treatable complications can contribute to the treatment discontinuation. <b><i>Methods:</i></b> We performed a 6-year prospective study of cutaneous reactions in 271 patients with chronic hepatitis C treated with peginterferon and ribavirin. <b><i>Results:</i></b> Cutaneous reactions of mild to moderate degree were seen in 36 (13.3%) patients: localized cutaneous reactions in 7 (2.6%) patients, generalized reactions - pruritus, skin xerosis and eczematous changes - in 28 (10.3%) patients, alopecia in 11 (4.1%) patients; exacerbation of lichen planus was seen in 1 patient. <b><i>Conclusion:</i></b> The study showed a relatively low prevalence of cutaneous reactions without the need for discontinuation of any of the drugs used. These reactions correlated only with the age of the patients and treatment duration. This finding is of particular importance with regard to the recently introduced direct-acting antivirals in the treatment of hepatitis C virus infection, which can cause a very severe form of cutaneous reactions.

Structure and Function of Human Stratum corneum under Deformation <i>Background:</i> The stratum corneum (SC) has an important barrier function. The effect of a mechanical stress applied to the SC is controversial on this important physiological parameter. <i>Objective and Methods:</i> To assess both in vitro and in vivo the structure and function of human SC submitted to controlled strains, we measured the transepidermal water loss (TEWL), in vivo, on human skin submitted to controlled strains ranging from 0 to 20% extension imposed by a Densi-score device. We also looked at the structure of the SC by means of X-ray diffraction and transmission electron microscopy. <i>Results:</i> Transmission electron microscopy and X-ray diffraction analysis were performed on harvested and stretched human SC. TEWL was not significantly influenced by the relative deformation applied to the skin. At high strain (60%) imposed in vitro to the SC, lipid bilayers and corneosomes were detached from corneocytes. Only rare corneosomes showed internal disruption. X-ray analysis did not reveal modifications in the supramolecular organization of intercellular lipids while stretching the SC. <i>Conclusion:</i> Submitting human SC to an extension force up to 20% elongation does not significantly alter the barrier function.
