Tóm tắt Đặt vấn đề Rheumatoid factor (RF) và xét nghiệm kháng thể peptide vòng citrullinated (anti-CCP) thường được đo vào thời điểm chẩn đoán viêm khớp dạng thấp (RA) nhưng có thể không được lặp lại và do đó không có sẵn trong dữ liệu hồ sơ sức khỏe điện tử (EHR); kết quả xét nghiệm lab cũng không có sẵn trong hầu hết các cơ sở dữ liệu yêu cầu hành chính. Mã ICD10 cho phép phân biệt giữa bệnh nhân có yếu tố huyết thanh dương tính (M05) (“dương tính với huyết thanh”) và bệnh nhân âm tính (M06), nhưng tính hợp lệ của các mã này chưa được kiểm tra. Phương pháp Sử dụng hệ thống thông tin viêm khớp của ACR (RISE) và dữ liệu U.S. MarketScan trong đó một số bệnh nhâ có kết quả xét nghiệm lab, chúng tôi đã xây dựng hai nhóm nghiên cứu. Sử dụng M05 làm proxy cho viêm khớp dạng thấp có yếu tố huyết thanh, độ nhạy = 0.76, PPV = 0.82 trong RISE, và Se = 0.73, PPV = 0.84 trong MarketScan. Kết quả cho M06 như một proxy cho viêm khớp dạng thấp không có yếu tố huyết thanh tương tự trong RISE, mặc dù hơi thấp hơn trong MarketScan. Trong 3 lần khám liên tiếp, khoảng 90% bệnh nhân RA đã được mã hóa nhất quán sử dụng hoặc M05 hoặc M06 tại mỗi lần khám. Kết luận Dưới ICD10, mã chẩn đoán M05 và M06 là các proxy hợp lý để xác định viêm khớp dạng thấp có và không có yếu tố huyết thanh với độ nhạy cao và giá trị dự đoán dương tính nếu kết quả xét nghiệm không có sẵn.
