Ý nghĩa tiên lượng của các khối u mediastinal lớn trong điều trị bệnh Hodgkin. Kinh nghiệm từ viện Gustave‐Roussy

Tóm tắtTại viện Gustave‐Roussy, chúng tôi đã tiến hành một nghiên cứu trên 154 bệnh nhân có giai đoạn lâm sàng I và II bệnh Hodgkin được điều trị bằng phương pháp xạ trị để đánh giá ý nghĩa tiên lượng của kích thước khối u mediastinal. Đối tượng nghiên cứu bao gồm những bệnh nhân được điều trị tại viện của chúng tôi và tham gia vào các thử nghiệm H2 và H5E.O.R.T.C. từ năm 1972 đến 1981.Bệnh nhân được chia thành ba nhóm để phục vụ cho phân tích: các khối u mediastinal lớn (tỷ lệ MT ≥ 0·35) được ghi nhận ở 20 trường hợp (13 phần trăm), xâm nhập mediastinal trung bình (tỷ lệ MT < 0·35) được quan sát ở 60 trường hợp (39 phần trăm), và 74 bệnh nhân (48 phần trăm) không có xâm nhập mediastinal khi khám. Tỷ lệ tái phát trong phổi là nhiều hơn ở những bệnh nhân có xâm nhập mediastinal khi khám (p < 0·001) nhưng không có sự khác biệt đáng kể về thống kê giữa những bệnh nhân có khối u ‘lớn’ và khối u ‘nhỏ’. Thêm vào đó, phân tích đa biến cho thấy cả sự hiện diện lẫn kích thước của bệnh mediastinal không ảnh hưởng bất lợi đến tỷ lệ sống sót không tái phát hay tỷ lệ sống sót toàn bộ.Các kết quả này có thể liên kết với kỹ thuật xạ trị, nơi mà kỹ thuật ‘chia liệu trình’ thường được sử dụng cho các khối u lớn và các vùng xạ thường được điều chỉnh trong quá trình điều trị dựa trên phản ứng của khối u. Do đó, chúng tôi không khuyến khích việc sử dụng thường xuyên điều trị ban đầu mở rộng như liệu pháp kết hợp dùng hóa trị liệu MOPP cho tất cả bệnh nhân có khối u mediastinal lớn. Thay vào đó, hóa trị liệu nên được dành riêng cho những khối u có kích thước đặc biệt lớn (tỷ lệ MT > 0·50) nơi xạ trị ban đầu có thể dẫn đến các biến chứng phổi muộn không đạt yêu cầu. Chúng tôi cũng khuyên sử dụng liệu pháp kết hợp cho những bệnh nhân có thể có một số yếu tố tiên lượng không thuận lợi mà chúng tôi đã xác định trước đây.
