Các rối loạn do rượu được quan sát ở cả ba hệ thống tế bào máu. Chúng xuất hiện khi tiêu thụ rượu lớn trong vài ngày và không phụ thuộc vào sự có mặt của bệnh xơ gan với lách to. Sau khi ngừng cung cấp rượu, các rối loạn này nhanh chóng và hoàn toàn hồi phục. Các rối loạn trong quá trình sản xuất hồng cầu biểu hiện trong tủy xương dưới dạng các tế bào megaloblast và ringsideroblast, cũng như sự hình thành chân không trong bào tương và nhân của các tiền thân hồng cầu và bạch cầu. Nguyên nhân là do thiếu axit folic và rối loạn hấp thụ sắt, có thể liên quan đến sự rối loạn tổng hợp heme do thiếu pyridoxalphosphate. Nồng độ sắt trong huyết thanh thường tăng cao trong trường hợp lạm dụng rượu và giảm nhanh chóng khi ngừng uống rượu.
