Sàng lọc chứng rối loạn phát triển khớp háng ở bệnh nhân chứng co cơ cổ bẩm sinh: có đủ chỉ cần khám lâm sàng?

Một mối liên quan giữa chứng co cơ cổ bẩm sinh (CMT) và chứng rối loạn phát triển của khớp háng (DDH) đã được thiết lập trong tài liệu; tuy nhiên, việc sàng lọc bệnh nhân có CMT để phát hiện DDH có cần thiết phải thực hiện hình ảnh khớp háng hay không vẫn còn gây tranh cãi. Mục đích của nghiên cứu này là xác định (1) tỷ lệ đồng tồn tại của DDH cần điều trị ở những cá nhân có CMT và (2) liệu chỉ khám lâm sàng có đủ để sàng lọc hay không. Một nghiên cứu hồi cứu dữ liệu đơn trung tâm đã được thực hiện trên 97 bệnh nhân liên tiếp từ ngày 1/1/2003 đến 1/9/2012 có CMT và đã thực hiện hình ảnh khớp háng. 12% (12/97) bệnh nhân có CMT bị DDH, tất cả đều cần điều trị. 75% (9/12) bệnh nhân có DDH có kết quả khám lâm sàng bất thường. Trong số ba bệnh nhân có DDH và kết quả khám lâm sàng bình thường, hai bệnh nhân đã đến để xin ý kiến thứ hai sau khi đã được điều trị DDH trước khi được đánh giá. 90% (9/10) bệnh nhân có DDH tại thời điểm trình diện có kết quả khám khớp háng bất thường. Tất cả 12 bệnh nhân có chứng loạn sản khớp háng đều được giới thiệu để theo dõi DDH hoặc DDH kèm theo CMT. Không có bệnh nhân nào được giới thiệu chỉ với CMT mà lại có DDH. Trong việc chăm sóc một bệnh nhân có CMT, điều quan trọng là các bác sĩ lâm sàng cần cảnh giác trong việc sàng lọc cho DDH. Một siêu âm hoặc chụp X-quang khớp háng nên được xem xét kỹ lưỡng như một phần trong đánh giá cho trẻ có CMT. Mức độ bằng chứng: IV.
