Việc hút thuốc có thể làm giảm hoặc thậm chí làm mất hoàn toàn hiệu quả của estrogen ngoại sinh và nội sinh. Điều này không chỉ làm giảm các tác động tích cực đối với các triệu chứng mãn kinh và chuyển hóa lipid, mà còn ảnh hưởng đến việc ngăn ngừa loãng xương và có thể là phòng ngừa các bệnh tim mạch. Đến nay, chỉ có estrogen dùng đường uống được chứng minh là chịu ảnh hưởng này. Nguyên nhân chủ yếu là do sự thanh thải qua gan tăng lên theo liều lượng, đặc biệt là trong bối cảnh nồng độ estrogen thấp. Việc thiếu hiệu ứng điều trị không nên được bù đắp bằng việc cung cấp liều cao hơn cho những người phụ nữ hút thuốc, vì có nguy cơ hình thành các chuyển hóa estrogen có khả năng gây đột biến, có thể làm tăng nguy cơ ung thư. Vì tác động tích cực của estrogen không bị giảm khi sử dụng estrogen qua da, nên hình thức ứng dụng này nên được ưu tiên cho những phụ nữ hút thuốc. Những người phụ nữ, mặc dù đã nhận được tất cả các cảnh báo, vẫn không muốn bỏ thuốc lá nên được thông báo rằng bên cạnh các tác động tiêu cực đã biết của thuốc lá, hiệu quả của liệu pháp hormone cũng có thể bị đặt ra nghi vấn.
