Mục đích của phân tích năm thứ hai được mô tả ở đây là để kiểm chứng liệu các kết quả tạm thời của năm đầu có duy trì suốt thời gian hai năm hay không.Phương phápThử nghiệm lâm sàng ngẫu nhiên, mù đôi, có kiểm soát HA trong 100 bệnh nhân OA đầu gối (98 ngẫu nhiên, 79 hoàn thành nghiên cứu) tại một đơn vị điều trị cấp ba có chuyên môn cao. Sự khác biệt về hiệu quả được giả định giữa PNHA và HA cho ước lượng cỡ mẫu ban đầu là 20%. Chu kỳ điều trị: ba lần tiêm khớp gối của PNHA hoặc HA, tại điểm chuẩn và hàng tuần trong hai tuần. Các đánh giá: điểm số Western Ontario và McMaster Universities (WOMAC) và điểm Knee Society Score (KSS) lần lượt là các điểm cuối chính và phụ, được đánh giá tại điểm chuẩn và sau 2, 6, 12 và 24 tháng; nồng độ chất trung gian trong dịch khớp (tại điểm chuẩn và cuối chu kỳ điều trị). Tác dụng phụ được điều tra trong mỗi lần kiểm tra. Phân tích thống kê: kiểm tra Kruskal-Wallis cho các mẫu độc lập (phân tích phương sai một chiều phi tham số) sau khi chỉnh sửa trung bình theo tuổi, chỉ số khối cơ thể và độ Kellgren-Lawrence. Nếu có ý nghĩa, so sánh nhiều cặp đôi Sidak sau đó.Kết quảĐiểm tổng KSS và mục đau KSS: cải thiện đáng kể ở cả hai nhóm, với cải thiện đau đáng kể hơn ở bệnh nhân được điều trị với PNHA (giảm 2 điểm) so với HA (giảm 1 điểm). Cả hai nhóm đều trải qua sự giảm điểm tổng WOMAC đáng kể trong thời gian dài: nổi trội hơn ở bệnh nhân điều trị bằng PNHA sau 24 tháng với sự khác biệt ổn định 16% nghiêng về PNHA trong tiểu mục đau WOMAC. Không có sự kiện có ý nghĩa lâm sàng nào trong cả hai nhóm.Kết luậnKết quả của nghiên cứu hai năm xác nhận rằng một chu kỳ điều trị ngắn bằng polynucleotide (tính năng bổ sung độ nhớt lâu dài, kích hoạt tế bào sụn, tính giảm đau) kết hợp cố định với acid hyaluronic có trọng lượng phân tử cao hiệu quả hơn trong việc cải thiện chức năng và giảm đau cho bệnh nhân OA đầu gối so với chỉ dùng acid hyaluronic.
