Đau đầu do lạm dụng thuốc (MOH) đã được công nhận là một vấn đề quan trọng ở bệnh nhân đau đầu mặc dù các cơ chế sinh lý bệnh vẫn chưa được làm rõ. Chẩn đoán MOH dựa trên các đặc điểm lâm sàng được xác định bởi Hiệp hội Đau Đầu Quốc tế. Mục tiêu của nghiên cứu này là đánh giá các tiêu chí chẩn đoán MOH trong một nhóm bệnh nhân đau mãn tính hỗn hợp nhằm thu thập thông tin về tỉ lệ mắc và các mối liên quan với MOH. Dữ liệu của tất cả bệnh nhân được giới thiệu đến phòng khám đau liên chuyên khoa tại Bệnh viện Đại học Zurich từ tháng 9 năm 2005 đến tháng 12 năm 2007 đã được phân tích hồi cứu. Các dữ liệu nhân khẩu học (tuổi, giới tính, lịch sử di cư), cũng như dữ liệu về thời gian bị đau, loại hình đau (thần kinh, tâm thần, thấp khớp, loại khác), việc sử dụng thuốc, tiền sử chấn thương và tình trạng đồng bệnh của trầm cảm và lo âu đã được thu thập. Tổng cộng có 178 trong số 187 bệnh nhân đau mãn tính liên tiếp được đưa vào nghiên cứu. Tổng cộng 138 bệnh nhân (78%) đã sử dụng thuốc giảm đau trên 15 ngày trong một tháng. Đau đầu mãn tính phổ biến hơn ở bệnh nhân lạm dụng thuốc giảm đau (39.8%) so với bệnh nhân không lạm dụng (18%). Tỉ lệ mắc MOH là 29%. Tỷ lệ odds (OR) cho một bệnh nhân có lạm dụng thuốc để mắc đau đầu mãn tính là 13.1 nếu họ có tiền sử đau đầu nguyên phát, so với một bệnh nhân không có hội chứng đau đầu nguyên phát. Hơn nữa, tiền sử đau đầu (OR 2.5, CI [1.13;5.44]), tiền sử di cư (OR 2.9, CI [1.31;6.32]) và trầm cảm đồng bệnh (OR 3.5, CI [1.46;8.52]) có liên quan đến lạm dụng thuốc khẩn cấp, nói chung. Đau đầu nguyên phát có nguy cơ cao bị mãn tính trong các bệnh nhân lạm dụng thuốc giảm đau cho các rối loạn đau khác. Trong khi đó, tiền sử đau đầu, tiền sử di cư và tình trạng đồng bệnh của trầm cảm có mối liên quan độc lập với lạm dụng thuốc giảm đau trong nhóm bệnh nhân này.
