Bệnh nhân béo phì nặng có nguy cơ cao hơn về thuyên tắc huyết khối tĩnh mạch (VTE) sau phẫu thuật giảm cân. Hóa dự phòng perioperative thường được sử dụng trong phẫu thuật giảm cân nhằm giảm nguy cơ VTE. Khi có hiện tượng chảy máu xảy ra, hóa dự phòng thường bị ngừng lại do lo ngại về việc có thể kích thích một sự kiện chảy máu khác. Chúng tôi đã tìm cách đánh giá mối quan hệ giữa chảy máu perioperative và VTE sau phẫu thuật trong phẫu thuật giảm cân. Dữ liệu từ Chương trình Cải tiến Chất lượng Phẫu thuật Quốc gia của Hội phẫu thuật Hoa Kỳ (NSQIP) trong giai đoạn 2012 đến 2014 đã được truy vấn để xác định bệnh nhân trải qua phẫu thuật giảm cân. Các phương pháp bao gồm phẫu thuật cắt dạ dày (n=28,145), cắt dạ dày ống (n=30,080), phẫu thuật giảm cân điều chỉnh (n=324) và phẫu thuật phân đoạn gan tuỵ (n=492). Phân tích hồi quy một biến và hồi quy đa biến đã được sử dụng để xác định các yếu tố perioperative dự đoán VTE sau phẫu thuật trong vòng 30 ngày ở những bệnh nhân có tình trạng chảy máu cần truyền máu. Tỷ lệ chảy máu cần truyền máu là 1,3%. Chảy máu có khả năng xảy ra cao hơn khi thực hiện phẫu thuật cắt dạ dày so với cắt dạ dày ống (1,6 so với 1,0%) (p<0,0001). Phân tích đa biến cho thấy tỷ lệ VTE cao hơn đáng kể sau khi truyền máu [Odds Ratio (OR) = 4,7; 95% CI 2,9–7,9; p<0,0001).
