Tóm tắt Đặt vấn đề Chất lượng của việc giảm gãy là một yếu tố quan trọng ảnh hưởng đến kết quả lâm sàng đối với gãy cổ xương đùi trật (FNFs). Tuy nhiên, vẫn còn những quan ngại về mức độ xâm lấn của phương pháp giảm mở và cố định bên trong (ORIF) so với phương pháp giảm đóng và cố định bên trong (CRIF), và sự lựa chọn giữa ORIF và CRIF như một chiến lược điều trị tối ưu cho gãy cổ xương đùi trật ở trẻ em vẫn đang gây tranh cãi. Phương pháp và vật liệu Chúng tôi đã tìm kiếm có hệ thống trong MEDLINE, Embase, và Thư viện Cochrane cho các nghiên cứu công bố đến ngày 22 tháng 12 năm 2022, so sánh các kỹ thuật ORIF và CRIF trong việc điều trị FNFs ở trẻ em. Phân tích tổng hợp đã xác định sự khác biệt trong kết quả phẫu thuật giữa ORIF và CRIF, đặc biệt về các biến chứng sau phẫu thuật, chẳng hạn như hoại tử xương đầu xương đùi (ONFH), không liền xương, biến dạng coxa vara, chênh lệch chiều dài chân (LLD), và đóng sớm khớp tăng trưởng (PPC). Kết luận Mặc dù có lo ngại về mức độ xâm lấn của ORIF, không có sự khác biệt về biến chứng giữa ORIF và CRIF sau FNFs ở trẻ em.
