NGHIÊN CỨU TÁC DỤNG DỰ PHÒNG RUN CỦA KETAMINE 0,2 MG/KG TIÊM TĨNH MẠCH SAU GÂY TÊ TỦY SỐNG

Mục tiêu: Đánh giá hiệu quả dự phòng run và các tác dụng không mong muốn khác của ketamine 0,2 mg/kg tiêm tĩnh mạch sau gây tê tủy sống (GTTS) cho phẫu thuật chi dưới. Phương pháp nghiên cứu: Nghiên cứu lâm sàng tiến cứu trên 30 bệnh nhân (BN) có chỉ định phẫu thuật chi dưới với phương pháp vô cảm GTTS với liều bupivacaine ưu tỷ trọng 0,5% 6 - 10mg kết hợp fentanyl 20mcg. Ngay sau GTTS, các BN được tiêm tĩnh mạch ketamine 0,2 mg/kg để dự phòng run. Kết quả: Tỷ lệ run trong mổ là 10%, trong đó, run độ 2 là 6,6% và run độ 3 là 3,3%. Tỷ lệ run sau mổ là 3,3% (run độ 2). Trong mổ, tỷ lệ ảo giác, buồn nôn, nôn, nhịp tim chậm, nhịp tim nhanh, tụt huyết áp, cao huyết áp, đau đầu, ngứa, chóng mặt, an thần với điểm an thần OAAS4 tương ứng là 26,7%; 6,6%; 3,3%; 36,7%; 3,3%; 73,3%; 3,3%; 3,3%; 6,6%; 26,7%. Sau mổ, tỷ lệ buồn nôn, nôn, đau đầu, ngứa tương ứng là 9,9%; 6,6%; 3,3%. Kết luận: Tiêm tĩnh mạch ketamine 0,2 mg/kg có hiệu quả dự phòng run sau GTTS cho phẫu thuật chi dưới với các tác dụng không mong muốn thoáng qua, dễ xử trí.
