Đau đầu do lạm dụng thuốc: một đánh giá quan trọng về các điểm kết thúc trong các nghiên cứu theo dõi gần đây

Không có hướng dẫn nào về việc thực hiện và trình bày kết quả của các nghiên cứu trên bệnh nhân mắc chứng đau đầu do lạm dụng thuốc (MOH). Mục tiêu của nghiên cứu này là xem xét các biện pháp kết quả dài hạn trong các nghiên cứu theo dõi được công bố từ năm 2006 trở đi. Chúng tôi đã bao gồm các nghiên cứu về MOH có thời gian >6 tháng và có ít nhất một điểm kết thúc đã được định nghĩa trước. Tổng cộng, chín nghiên cứu đã được xác định. 1.589 bệnh nhân MOH (22% là nam) có tần suất trung bình chung là 25,3 ngày đau đầu/tháng tại thời điểm ban đầu. Số ngày đau đầu/tháng tại thời điểm kết thúc theo dõi được báo cáo trong sáu nghiên cứu (trung bình 13,8 ngày/tháng). Sự giảm sút rõ nét hơn đối với các nghiên cứu bao gồm bệnh nhân chỉ mắc chứng đau đầu migraine (−14,6 ngày/tháng) so với các nghiên cứu có chẩn đoán ban đầu là migraine và đau đầu loại căng thẳng (−9,2 ngày/tháng). Sáu nghiên cứu báo cáo tỷ lệ tái phát (trung bình 26%) và/hoặc tỷ lệ người đáp ứng (trung bình 28%). Số ngày sử dụng thuốc/tháng và sự thay đổi ở chỉ số đau đầu tại thời điểm kết thúc theo dõi chỉ được báo cáo trong một và hai trong chín nghiên cứu, tương ứng. Đánh giá hiện tại cho thấy sự thiếu đồng nhất về các điểm kết thúc được sử dụng trong những nghiên cứu theo dõi được công bố gần đây. Cần có hướng dẫn để trình bày dữ liệu theo dõi về MOH và chúng tôi đề xuất các điểm kết thúc như số ngày đau đầu/tháng, số ngày sử dụng thuốc/tháng, tỷ lệ tái phát và tỷ lệ người đáp ứng được định nghĩa là giảm ≥50% tần suất đau đầu và/hoặc chỉ số đau đầu so với mức ban đầu.
