Klarzelltumor der Lunge

Der Klarzelltumor der Lunge ist ein sehr seltener, meist gutartiger Tumor aus epitheloiden, HMB45-positiven Tumorzellen. Als Ursprungszelle gilt die perivaskuläre epitheloide Zelle, die histogenetisch auch bei Angiomyolipomen relevant ist. Aufgrund seines klarzelligen Charakters ist die Abgrenzung dieses Tumors insbesondere von klarzelligen Malignomen und deren Metastasen schwierig. Klinik und bildgebende Diagnostik sind in der Regel nicht wegweisend, sodass erst anhand der Histomorphologie und Immunhistochemie eine eindeutige Diagnose gestellt werden kann. Es wird der Fall einer 52-jährigen Patientin mit einem unklaren Rundherd im rechten Lungenunterlappen beschrieben. Es erfolgte eine explorative Thorakotomie, im Schnellschnitt wurde zunächst ein solide trabekulär wachsender hellzelliger Tumor, nicht typisch für einen epithelialen Primärtumor in der Lunge, in der Paraffinhistologie ein Klarzelltumor der Lunge diagnostiziert. Die Schwierigkeit der korrekten Diagnose des Klarzelltumors der Lunge im Rahmen der Schnellschnittuntersuchung wird anhand des hier vorliegenden Beispiels dargestellt, ebenso wird die Differenzialdiagnostik dieses Tumors diskutiert.
