Chứng suy yếu là một hội chứng phức tạp, đa chiều liên quan đến tuổi tác, làm tăng khả năng nhạy cảm với các hậu quả sức khỏe bất lợi và chất lượng cuộc sống kém. Có sự đồng thuận ngày càng tăng về việc sự viêm mãn tính và thay đổi hệ thống miễn dịch góp phần vào chứng suy yếu, tuy nhiên vai trò rõ ràng của những thay đổi này vẫn cần được làm sáng tỏ. Hơn nữa, các cytokine gây viêm và chống viêm cùng với các phân tử tín hiệu khác có thể lan truyền từ các tế bào đã được kích hoạt đến các tế bào liền kề thông qua các vesicle ngoại bào (EVs), mà chúng cũng đóng vai trò trong quá trình lão hóa tế bào. Mục tiêu của nghiên cứu hiện tại là điều tra xem EVs có đóng vai trò trong chức năng miễn dịch ở chứng suy yếu hay không. Trong 219 người lớn tuổi từ 76 đến 78 tuổi, được chọn từ nghiên cứu InveCe.Ab (Abbiategrasso, Ý), chúng tôi đã điều tra tình trạng viêm và giao tiếp giữa các tế bào do EVs. Protein phản ứng C (CRP) và các cytokine gây viêm (IL-1β, IL-2, IL-6, IL-8, IL-12 p70, TNFα và IFNγ) và chống viêm (IL-4, IL-10, IL-13) đã được đánh giá trong huyết tương của các đối tượng Suy yếu và không Suy yếu. Chúng tôi ghi nhận sự tăng đáng kể của CRP, interleukin-1β và -6 (IL-1β, IL-6) cùng với mức độ protein hoại tử khối u alpha (TNFα) trong huyết tương ở chứng suy yếu. Ở những người phụ nữ có chứng suy yếu, chúng tôi cũng ghi nhận sự tăng lên của interferon -gamma (IFN -γ) và, ngạc nhiên là, IL-13, một cytokine chống viêm, sự tăng lên này dường như đối lập với lý thuyết viêm lão hóa. Một panel viêm (các thụ thể toll-like 2 và 4 (TLR2 và TLR4), các thụ thể protein hoại tử khối u TNFRec5/CD 40 và TNFRec1B/CD120B) và một panel bao gồm các thụ thể liên quan đến sự lão hóa tế bào (thụ thể yếu tố tăng trưởng giống insulin 1 (CD221) và thụ thể interleukin 6 (IL-6R)) đã được phân tích trong các EVs lớn (lEVs) tách từ huyết tương của các đối tượng Suy yếu (n = 20) và không Suy yếu (n = 20). Trong các lEVs tách từ huyết tương của các đối tượng Suy yếu, chúng tôi ghi nhận sự gia tăng của TLR2 và TLR4, TNFRec5/CD 40 và TNFRec1B/CD120B, cho thấy một trạng thái viêm mãn tính. Ngoài ra, sự gia tăng CD221 và IL-6R trong lEVs của các cá thể Suy yếu. Kết luận, tình trạng viêm gây ra, đặc biệt là sự gia tăng các cytokine tuần hoàn là rất quan trọng để hiểu các cơ chế tiềm tàng đối với hội chứng suy yếu. Hơn nữa, sự giải phóng cytokine từ EVs, chủ yếu là các EVs lớn, vào không gian ngoại bào cho thấy sự đóng góp của chúng vào việc hình thành một môi trường viêm và lão hóa, điều này có thể góp phần vào chứng suy yếu.
