Chỉ số khối cơ thể thấp (BMI) ở những người cao tuổi có chức năng thận giảm là điều quan trọng vì liên quan đến tiên lượng kém và tình trạng suy yếu. Trong nghiên cứu này, chúng tôi nhằm làm rõ mối liên hệ giữa BMI và tỷ lệ tử vong trong bệnh viện ở những bệnh nhân cao tuổi mắc bệnh thận mãn tính không phụ thuộc vào thẩm tách (CKD) được phân loại theo chức năng thận. Sử dụng dữ liệu từ Cơ sở dữ liệu Tầm nhìn Y tế, nghiên cứu đoàn hệ đa trung tâm này bao gồm những người từ 60 tuổi trở lên có mức độ lọc cầu thận ước tính dưới 60 ml/phút/1.73 m2 nhưng không phụ thuộc vào thẩm tách, đã nhập viện vì viêm phổi do vi khuẩn trong giai đoạn 2014–2019. Chúng tôi so sánh nguy cơ tử vong trong bệnh viện giữa các bệnh nhân với các loại BMI dựa trên các phân vị (thấp, trung bình–thấp, trung bình–cao và cao), coi BMI trung bình–cao là tham chiếu. Chúng tôi cũng đánh giá mối liên hệ với BMI bằng cách sử dụng spline bậc ba, coi BMI là một biến liên tục phi tuyến và BMI 22 kg/m2 là tham chiếu. Chúng tôi cũng đánh giá mối liên hệ giữa BMI và chức năng thận bằng một mô hình tổng quát tính thêm các thành phần tương tác giữa BMI liên tục phi tuyến và chức năng thận. Chúng tôi thu thập dữ liệu cho 3,952 bệnh nhân, với 350 (8.9%) trường hợp tử vong trong bệnh viện. So với BMI trung bình–cao, BMI thấp liên quan đến nguy cơ tử vong cao hơn và thời gian nằm viện lâu hơn, trong khi hai phân loại còn lại thì tương đương. Các mô hình sử dụng spline bậc ba cho thấy mối liên hệ giữa BMI và tử vong trong bệnh viện cho thấy một đường cong hình chữ L; BMI < 22.0 kg/m2 liên quan đến nguy cơ tử vong cao hơn, và tại BMI 18.5 kg/m2, tỷ lệ odds là 1.43 với khoảng tin cậy 95% từ 1.26 đến 1.61 so với BMI 22.0 kg/m2. Phân tích các hiệu ứng tương tác của chức năng thận bằng mô hình tổng quát cho thấy rằng mối liên hệ bảo vệ của BMI cao giảm dần theo sự suy giảm chức năng thận. Nghiên cứu đoàn hệ này gợi ý rằng không chỉ BMI thấp và chức năng thận kém đều độc lập liên quan đến tỷ lệ tử vong trong bệnh viện mà cũng cho thấy tác động bảo vệ của BMI cao yếu đi khi chức năng thận giảm thông qua việc phân tích các thành phần tương tác. Nghiên cứu này nhấn mạnh sự cần thiết phải phòng ngừa thiếu cân và chứng minh mối tương tác giữa BMI và chức năng thận ở những bệnh nhân cao tuổi mắc CKD không phụ thuộc vào thẩm tách.
