Gãy xương cánh tay khuỷu là loại gãy xương phổ biến nhất ở khuỷu tay trẻ em. Thông thường, việc giảm kín và chốt xuyên da có thể mang lại kết quả đáng hài lòng sau khi giảm đầy đủ. Góc Baumann thường được sử dụng để đánh giá chất lượng giảm, tuy nhiên, nó có thể không đánh giá tốt khi khuỷu tay ở tư thế gập và/hoặc khi bệnh nhân còn nhỏ. Chúng tôi đã thực hiện nghiên cứu này để đánh giá giá trị tiềm năng của góc khủy tay xương cánh tay (HTa) trong việc đánh giá phần giảm và so sánh nó với góc Baumann. Chúng tôi đã xem xét hồi cứu các trường hợp gãy xương cánh tay khuỷu tại trung tâm chấn thương của chúng tôi từ năm 2016 đến 2019. Các bệnh nhân được phân loại như sau: trong nhóm HTa, một phương pháp chụp khớp đã được sử dụng để đánh giá góc HTa và sự giảm (HTa, được định nghĩa bởi giao điểm của trục thân xương cánh tay và tiếp tuyến của bề mặt khớp của xương lồi cầu); trong nhóm Baumann, góc Baumann được sử dụng để đánh giá sự giảm. Tỷ lệ góc Baumann (BA của bên bị thương/ BA của bên đối diện) được tính toán để đánh giá chất lượng giảm giữa các nhóm. Tiêu chí phân loại của Flynn được sử dụng để đánh giá cả hai kết quả chức năng và thẩm mỹ trong hai nhóm trong thời gian theo dõi. Thời gian phẫu thuật, số lần chụp fluoroscopy, biến chứng và điểm phân loại Flynn của các nhóm được so sánh với nhau. Tổng cộng có 57 bệnh nhân với độ tuổi trung bình là 4,62 năm và thời gian theo dõi là 21,49 ± 5,40 tháng được đưa vào phân tích. Phân bố giới tính và độ tuổi tương tự ở hai nhóm. Số lần chụp fluoroscopy trong nhóm HTa thấp hơn đáng kể so với nhóm Baumann (16,17 ± 0,73 so với 21,85 ± 0,78, giá trị p < 0,0001), và thời gian phẫu thuật cũng ít hơn ở nhóm HTa (45,78 ± 1,96 phút so với 62,21 ± 1,58 phút, giá trị p < 0,0001). Tỷ lệ góc Baumann không cho thấy sự khác biệt đáng kể giữa hai nhóm (1,002 ± 0,023 ở nhóm Baumann và 1,01 ± 0,023 ở nhóm HTa, giá trị p < 0,0001). Thời gian nằm viện, biến chứng và điểm phân loại Flynn tương tự nhau ở các nhóm. Góc HTa có mối tương quan dương với góc Baumann trong nhóm HTa (giá trị R là 0,71 và P = 0,0002). Không có sự khác biệt đáng kể về chất lượng giảm và điểm Flynn giữa nhóm HTa và nhóm Baumann. Hơn nữa, HTa liên quan đến thời gian phẫu thuật ngắn hơn và ít phơi nhiễm bức xạ trong nghiên cứu này. Do đó, HTa có thể là một thông số thuận tiện và đáng tin cậy để hướng dẫn việc giảm gãy xương cánh tay khuỷu, đặc biệt là đối với trẻ nhỏ.
