Lập kế hoạch chăm sóc trước (ACP), một quá trình giao tiếp về sở thích của bệnh nhân đối với chăm sóc y tế trong tương lai, cần được khởi xướng kịp thời. Thực hành gia đình (GP) có vị trí lý tưởng để bắt đầu quá trình này. Can thiệp nhằm cải thiện việc khởi động ACP cho bệnh nhân mắc bệnh mãn tính, giới hạn sự sống tại cơ sở thực hành y tế gia đình (ACP-GP) bao gồm một cuốn sổ làm việc ACP cho bệnh nhân, đào tạo về giao tiếp ACP cho các GP, các cuộc trò chuyện ACP được lên kế hoạch và tài liệu hóa kết quả cuộc trò chuyện ACP trong một mẫu cấu trúc.
