Tóm tắt Đặt vấn đề Sự bùng phát của bệnh coronavirus năm 2019 (COVID-19) hiện đang trở thành một mối đe dọa lớn đối với sức khỏe cộng đồng. Quá trình lâm sàng của COVID-19 rất rộng, trong đó các ca bệnh nghiêm trọng có sự tiến triển bệnh nhanh chóng và tỷ lệ tử vong cao. Mục tiêu của nghiên cứu của chúng tôi là tóm tắt các đặc điểm của các kiểu bệnh khác nhau và khám phá các yếu tố nguy cơ gây nặng bệnh để nhận diện sớm và điều trị kịp thời. Phương pháp Trong nghiên cứu hồi cứu này, chúng tôi đã thu thập dữ liệu của các bệnh nhân được xác nhận mắc COVID-19 tại tỉnh Chiết Giang từ 17 tháng 1 đến 12 tháng 2 năm 2020. Theo định nghĩa phân loại lâm sàng, chúng tôi đã chia các trường hợp xác nhận thành bốn loại và tóm tắt các đặc điểm về dịch tễ học, lâm sàng, kết quả xét nghiệm và chụp X-quang, cũng như các phương pháp điều trị và kết quả, tương ứng. Hơn nữa, chúng tôi đã sử dụng các mô hình hồi quy logistic thứ bậc đơn biến và đa biến để khám phá các yếu tố nguy cơ gây nặng bệnh ở bệnh nhân COVID-19. Kết quả Tổng cộng có 788 bệnh nhân được tham gia nghiên cứu của chúng tôi, trong đó 52 ca (6.6%) thuộc loại nhẹ, 658 ca (83.5%) thuộc loại thông thường, 61 ca (7.2%) thuộc loại nặng, và 17 ca (2.2%) thuộc loại nghiêm trọng. Hồi quy logistic đa biến cho thấy khả năng nặng bệnh ở bệnh nhân COVID-19 có liên quan đến giới tính nam (tỷ lệ odds [OR] = 1.7, khoảng tin cậy 95% [CI]: 1.2–2.6 P = 0.008), sốt (OR = 3.6, 95% CI: 2.1–6.3, P <  0.001), ho (OR = 1.7, 95% CI: 1.0–2.9, P = 0.041), khạc ra máu (OR = 3.4, 95% CI: 1.1–10.3, P = 0.032), triệu chứng tiêu hóa (OR = 1.9, 95% CI: 1.0–3.5, P = 0.047), huyết áp cao (OR = 2.6, 95% CI: 1.2–5.6, P = 0.013). Với sự gia tăng của độ tuổi, nguy cơ nặng bệnh dần dần cao hơn (≤ 18 tuổi [OR = 1.0], 19–40 tuổi [OR = 12.7, 95% CI: 4.5–36.0, P < 0.001], 41–65 tuổi [OR = 14.8, 95% CI: 5.2–42.1, P <  0.001], ≥ 66 tuổi [OR = 56.5, 95% CI: 17.1–186.5, P < 0.001]). Kết luận Các bác sĩ lâm sàng cần chú ý đến những đặc điểm này ở bệnh nhân COVID-19, bao gồm độ tuổi cao, giới tính nam, sốt, ho, khạc ra máu, triệu chứng tiêu hóa và huyết áp cao để nhận diện sớm mức độ nghiêm trọng của bệnh.
