Tóm tắt

Tóm tắtÝ nghĩa lâm sàng và tiên lượng của phân loại Kiel đối với u lympho không Hodgkin (NHL) đã được nghiên cứu trên 1127 bệnh nhân tham gia vào một nghiên cứu theo lối quan sát đa trung tâm có hướng prospect. Tỷ lệ sống sót của 782 (69,4 phần trăm) bệnh nhân mắc NHL ác tính độ thấp (u lympho lymphocytic, chủ yếu là B-CLL, LP immunocytoma, u lympho centrocytic, u lympho centroblastic-centrocytic) cao hơn so với 341 bệnh nhân (30,2 phần trăm) mắc NHL ác tính độ cao (u lympho centroblastic, immunoblastic, lymphoblastic). Tiên lượng tốt nhất ở những bệnh nhân mắc u lympho centroblastic-centrocytic và B-CLL, trong khi kém thuận lợi nhất ở u lympho immunoblastic và lymphoblastic. Tỷ lệ sống sót của bệnh nhân LP immunocytoma và u lympho centrocytic nằm ở mức trung gian sau 2 đến 2,5 năm theo dõi. Tương ứng với bệnh lý mô học, hình dạng của các đường cong sống sót của NHL ác tính độ thấp (giảm chậm, không có bình nguyên) khác biệt so với NHL ác tính độ cao (giảm nhanh, sau đó có bình nguyên). Tiên lượng của B-CLL tốt hơn so với LP immunocytoma. Các giai đoạn I và II thường xảy ra nhiều hơn ở những bệnh nhân mắc u lympho centroblastic-centrocytic (21 phần trăm) so với LP immunocytoma (2,5 phần trăm) và u lympho centrocytic (11 phần trăm). Khả năng của xạ trị để tạo ra các đáp ứng hoàn toàn ổn định ở giai đoạn III của u lympho centroblastic-centrocytic chỉ ra rằng có sự kìm hãm kéo dài của bệnh lý vào hệ bạch huyết. Trong các loại u lympho immunoblastic và centroblastic, các giai đoạn I và II đã được chẩn đoán ở 34 và 38 phần trăm bệnh nhân tương ứng, nhưng chỉ ở giai đoạn I/IE của u lympho centroblastic là các đáp ứng kéo dài đạt được bằng xạ trị. Ở các bệnh nhân mắc NHL ác tính độ cao giai đoạn muộn, cải thiện đáng kể về tiên lượng chỉ có thể thực hiện được thông qua việc tạo ra các đáp ứng hoàn toàn, trong khi trong các u lympho ác tính độ thấp tương ứng, ngay cả các đáp ứng một phần cũng có ý nghĩa về tiên lượng.
