Tắc nghẽn vi mạch (MO) hay hiện tượng không chảy máu là một biến chứng đã được xác nhận của liệu pháp tái tưới máu trong nhồi máu cơ tim cấp. Nó ngày càng được nhìn nhận như một chỉ số tiên lượng kém và là dấu hiệu của sự tái cấu trúc thất trái không thuận lợi sau này. Mặc dù MO có thể được đánh giá bằng nhiều phương pháp hình ảnh khác nhau bao gồm điện tâm đồ, siêu âm tim có chất tương phản, chụp xạ hình hạt nhân và chụp động mạch vành, nhưng việc đánh giá bằng cộng hưởng từ tim mạch (CMR) đặc biệt hữu ích trong việc nâng cao khả năng phát hiện, chẩn đoán và định lượng, cũng như theo dõi các tác động tiếp theo của nó lên quá trình tiến triển và phục hồi của vùng nhồi máu. Đánh giá MO đã trở thành một phần thường quy trong việc đánh giá CMR của nhồi máu cơ tim cấp và sẽ ngày càng đóng vai trò quan trọng trong các thử nghiệm lâm sàng về các tác nhân và chiến lược tái tưới máu bổ sung. Bài tổng quan này sẽ tóm tắt sinh lý bệnh của MO, các phương pháp CMR hiện tại dùng để chẩn đoán, các ý nghĩa lâm sàng, và những hướng đi trong tương lai cần thiết để cải thiện hiểu biết của chúng ta về vấn đề lâm sàng phổ biến này.
