AbstractTwo cases of fatal acute necrotizing pancreatitis shortly after distal splenorenal shunt are presented. Instrumental injury to the pancreas during operation may have caused this complication. It is suggested that distal splenorenal shunt should not be performed when mobilization of the splenic vein is technically difficult.
