Predictors of Sexual Recidivism: Did Meta-Analysis Clarify the Role and Relevance of Denial?

Seven studies selected for meta-analysis of the denial variable in a large-scale meta-analysis of predictors of sexual recidivism are briefly reviewed and a number of methodologic issues are identified that relate to interpretation of the failure of meta-analysis to find an effect for the denial variable. These include variability in definition of the denial variable, variation in treatment access and exclusion for deniers, low base rates of recidivism, small sample sizes, low power, and high probability of type II error across individual studies. Despite the apparent objectivity and power of this methodology, a careful examination of the individual studies suggests that meta-analysis did not clarify the role and relevance of denial as a predictor of sexual recidivism. Further work is necessary to clarify the role and relevance of denial in treatment success, risk assessment, and risk prediction.
Different Actuarial Risk Measures Produce Different Risk Rankings for Sexual Offenders

Percentile ranks were computed for N=262 sex offenders using each of 5 actuarial risk instruments commonly used with adult sex offenders (RRASOR, Static-99, VRAG, SORAG, and MnSOST-R). Mean differences between percentile ranks obtained by different actuarial measures were found to vary inversely with the correlation between the actuarial scores. Following studies of factor analyses of actuarial items, we argue that the discrepancies among actuarial instruments can be substantially accounted for by the way in which the factor Antisocial Behavior and various factors reflecting sexual deviance are represented among the items contained in each instrument. In the discussion, we provide guidance to clinicians in resolving discrepancies between instruments and we discuss implications for future developments in sex offender risk assessment.
A Clinical Model for the Treatment of Personality Disordered Sexual Offenders: An Example of Theory Knitting

In this paper we present a clinical model for the treatment of personality disordered sexual offenders. The model is based on the theoretical assumption that cognitive distortions emerge from more general explanatory theories. It seems that the pathways of offending processes that are characterized by positive or negative affects can be found in all treated sex offenders regardless of their motivation. We assume that these pathways are activated by cognitive distortions that illustrate the underlying general explanatory theory held by the offender and that these distortions are typical for overall daily interpersonal behavior of the offender. This means that, as predicted by our clinical model in the case of offenders with a personality disorder, the type of explanatory cognition that is underlying their sexual offenses is the same as the one that is underlying their general explanatory theories. We will describe the clinical model and the treatment program used at Forensic Psychiatric Hospital Center Veldzicht with a review of our experiences.
Pathways in the offending process of extrafamilial sexual child molesters

The aim of the current study was to investigate specific pathways in the offending process of extrafamilial sexual child molesters. Forty-four men who had committed at least one sexual offense against a nonfamilial prepubescent child were included in this study and were classified using cluster analysis. Subjects using the coercive pathway (n=30) had generally used psychoactive substances before their offenses. Furthermore, they had molested a female victim without perceived vulnerability and whom they had already well known. These molesters had not planned their offense, which was of short duration (less than 15 min), and involved coital activities and coercion (verbal and/or physical). Subjects using the noncoercive pathway (n=14) had generally used pornography and deviant sexual fantasies before their offenses. Moreover, they had molested a male victim, in whom they perceived a psychosocial vulnerability and who was not familiar to them. These molesters had planned their offense, which was of longer duration (more than 15 min) and involved noncoital activities without coercion. These two pathways were compared to the two pathways in the offending process identified by Ward and his colleagues.
Comparing Personality Characteristics of Juvenile Sex Offenders and Non-Sex Offending Delinquent Peers: A Preliminary Investigation

There exists a paucity of research on psychopathology among Juvenile Sex Offenders (JSOs) as measured by standardized instruments, particularly as compared to nonclinical and other clinical populations. The current investigation involved a comparison of scores on the Minnesota Multiphasic Personality Inventory—Adolescent Version (MMPI-A) between a group of JSOs and non-sex offending delinquent peers. Thirty-six youth from a staff-secure residential facilitate participated, 18 JSOs and 18 non-sex offending delinquent peers. Results revealed that the mean score on clinical Scale 4 (Psychopathic Deviant) was in the clinically significant range for non-sex offending peers and not for JSOs, although the difference between the mean scores was not statistically significant. Chi-square analysis indicated that a significantly greater number of non-sex offending delinquents had scores in the clinical range on this scale as compared to JSOs. Results are discussed in the context of current research on psychopathology among JSOs.
Therapist Style in Sexual Offender Treatment: Influence on Indices of Change

This paper describes the results of our review of the broad process literature as well as summarizing two studies examining the influence of the therapist in generating changes in treatment targets among sexual offenders. We conclude that displays of empathy and warmth by the therapist as well as the provision of rewards for progress and some degree of directiveness, maximize the benefits derived from the procedures employed in treating sexual offenders. We also suggest the need for flexibility and encourage program designers not to so overly detail their treatment manuals that the influence of the therapist is minimized.

Predictors of Recidivism in Australian Juvenile Sex Offenders: Implications for Treatment

Juvenile sex offenders charged with their first sexual offence were compared with recidivist juvenile sex offenders who had been charged with more than one sexual offence on a number of factors related to sexual offending. Participants were 70 male juvenile sex offenders, aged 13–21 years who were awaiting court disposition. Negative family history, negative family characteristics, school and learning problems, social skill deficits, deviant sexual experiences, deviant sexual fantasies, and cognitive distortions were assessed for their direct and mediating roles in recidivism. Path analysis indicated that poor social skills, learning problems, and deviant sexual experiences were causally related to recidivism of sexual offending. Poor social skills were directly related to recidivism, whereas cognitive distortions and deviant sexual fantasies mediated the role of learning problems and deviant sexual experiences. There was a significant association between deviant sexual experience and learning problems. The findings support the role of cognitive distortions and deviant sexual fantasies in recidivist sexual offending for this sample. The causal role identified for poor social skills and learning problems in recidivism for sexual offending has implications for treatment and therefore deserves further attention.
Attitudes About Community Notification: A Comparison of Sexual Offenders and the Non-offending Public

Public notification laws have been enacted by legislatures to address the risk posed by sex offenders to the public. Little research has been done regarding either the public’s or sex offenders’ knowledge and attitudes about community notification and its impact. This study compared the experiences and perceptions of 125 sex offenders in outpatient treatment to 193 members of the public in Brevard County, Florida. Sex offenders were significantly more likely to believe that community notification laws and society are unfair, to feel that such laws should be applied to fewer sex offenders based on their level of risk or threat, and to view notification as being ineffective at reducing sex crimes. Nearly half of the offenders reported experiencing threats, property damage, or physical assault as a result of public disclosure. In comparison, only 10% of the public was aware of vigilantism against sex offenders. Implications for public policy and future research are discussed.
