The role of rewards and demands in burnout among surgical nurses

Job rewards have both, an intrinsic and an extrinsic motivational potential, and lead to employees’ development as well as help them to achieve work goals. Rewards can balance job demands and protect from burnout. Due to changes on the labour market, new studies are needed. The aim of our study was to examine the role of demands and individual rewards (and their absence) in burnout among surgical nurses. The study was conducted in 2009 and 2010 with 263 nurses who worked in surgical wards and clinics in hospitals in Southern Poland. The hypotheses were tested by the use of measures of demands and rewards (Effort-Reward Imbalance Questionnaire by Siegrist) and burnout syndrome (Maslach Burnout Inventory). A cross-sectional, correlational study design was applied. Nurses experienced the largest deficiencies in salary and prestige. Exhaustion was explained by stronger demands and lack of respect (large effect). Depersonalization was explained by stronger demands, lack of respect and greater job security (medium effect). Reduced personal achievement was explained by more demands and greater job security (small effect). Excessive demands and lack of esteem are key reasons for burnout among surgical nurses. Job security can increase burnout when too many resources are invested and career opportunities do not appear. These results may help to improve human resource management in the healthcare sector.
A proposal for calculating the no-observed-adverse-effect level (NOAEL) for organic compounds responsible for liver toxicity based on their physicochemical properties

Both environmental and occupational exposure limits are based on the no-observed-adverse-effect level (NOAEL), lowest-observed-adverse-effect level (LOAEL) or benchmark dose (BMD) deriving from epidemiological and experimental studies. The aim of this study is to investigate to what extent the NOAEL values for organic compounds responsible for liver toxicity calculated based on their physicochemical properties could be used for calculating occupational exposure limits. The distribution coefficients from air to the liver (log Kliver) were calculated according to the Abraham solvation equation. NOAEL and LOAEL values for early effects in the liver were obtained from the literature data. The descriptors for Abraham’s equation were found for 59 compounds, which were divided into 2 groups: “non-reactive” (alcohols, ketones, esters, ethers, aromatic and aliphatic hydrocarbons, amides) and “possibly reactive” (aldehydes, allyl compounds, amines, benzyl halides, halogenated hydrocarbons, acrylates). The correlation coefficients between log-log K and log NOAEL for non-reactive and reactive compounds amounted to r = −0.8123 and r = −0.8045, respectively, and were statistically significant. It appears that the Abraham equation could be used to predict the NOAEL values for compounds lacking information concerning their liver toxicity. In view of the tendency to limit animal testing procedures, the method proposed in this paper can improve the practice of setting exposure guidelines for the unstudied compounds.
Professional experience and ergonomic aspects of midwives’ work

The most frequent manner of attending childbirth imposes on midwives assuming poor body position affecting the musculoskeletal system. Long professional experience does not mitigate the negative effects. The adopted movement habit, as well as the type, number and frequency of actions influence the body posture. The aim of the study was to identify ergonomic threats of basic occupational midwives activities and how particular spinal segments arrangements while attending childbirth using the same technique in senior midwives differ from those of junior ones. It was also checked whether pain influences the working position assumed by midwives. Examinations were conducted in 95 midwives aged 21–50 (X = 29.25±9.34): 51 graduates of BSc midwifery who worked 680 h in delivery rooms during obligatory practical classes and apprenticeship and 44 senior midwives with professional experience of 7–27 years (X = 14.84±5.98). The study was threefold. The spinal alignment while performing work activities associated with attending childbirth was assessed using the OWAS system and the SonoSens Monitor, the center of gravity projection on basal plane — using the AccuGait AMTI stabilometric platform. The measurements were taken during a simulation of attending childbirth (on examination model). A survey was conducted aimed at identifying spinal pain. Midwives’ working postures require unnatural body alignments. Postural instability in the working position and no maximal usage of basal plane were observed. The work overload may afflict the musculoskeletal system, which was confirmed by different pain discomforts in 67.3% of the examinees. Spinal alignment while attending childbirth is individually differentiated and in every case non-ergonomic. Identifying explicitly spinal overloads is difficult, but the most prevalent ones affect lumbar and cervical regions altogether. Spinal pain is frequently noted, both in junior and senior midwives, and is characteristic for midwives working in maximal movement ranges.

Voice amplification for primary school teachers with voice disorders: A randomized clinical trial

Several studies have demonstrated a high prevalence of voice disorders in teachers, together with the personal, professional and economical consequences of the problem. Good primary prevention should be based on 3 aspects: 1) amelioration of classroom acoustics, 2) voice care programs for future professional voice users, including teachers and 3) classroom or portable amplification systems. The aim of the study was to assess the benefit obtained from the use of portable amplification systems by female primary school teachers in their occupational setting. Forty female primary school teachers attended a course about professional voice care, which comprised two theoretical lectures, each 60 min long. Thereafter, they were randomized into 2 groups: the teachers of the first group were asked to use a portable vocal amplifier for 3 months, till the end of school-year. The other 20 teachers were part of the control group, matched for age and years of employment. All subjects had a grade 1 of dysphonia with no significant organic lesion of the vocal folds. Most teachers of the experimental group used the amplifier consistently for the whole duration of the experiment and found it very useful in reducing the symptoms of vocal fatigue. In fact, after 3 months, Voice Handicap Index (VHI) scores in “course + amplifier” group demonstrated a significant amelioration (p = 0.003). The perceptual grade of dysphonia also improved significantly (p = 0.0005). The same parameters changed favourably also in the “course only” group, but the results were not statistically significant (p = 0.4 for VHI and p = 0.03 for perceptual grade). In teachers, and particularly in those with a constitutional weak voice and/or those who are prone to vocal fold pathology, vocal amplifiers may be an effective and low-cost intervention to decrease potentially damaging vocal loads and may represent a necessary form of prevention.
Thirty years of evolution of oral health behaviors in the working-age Poles

In an average Polish person aged 35–44, more than 16 teeth have or had been affected by dental caries. Of that number, almost half of the teeth have already been extracted. Oral health behaviours contribute to this civilization disease in 50%. Such poor oral health status limits the ability of the affected people to take many social and professional roles. To evaluate current oral health behaviours and their trends among 35–44 year old Polish people during the period of recent 30 years. The data were obtained from the International Collaborative Studies conducted in 1978 and 1988 at the Nofer Institute of Occupational Medicine, Łódź, Poland, under the auspices of the World Health Organization (WHO) as well as from 3 stages of the study on Nationwide Monitoring of Oral Health Status and Its Conditioning performed in 1998, 2002 and 2010. The researchers evaluated oral health behaviours and oral health condition of 5425 subjects. Despite a noticeable improvement, poor oral health behaviours are observed in 30%–40% of the adults. In the analysed period, the number of people brushing their teeth at least twice a day increased by more than 10% and the number of people using dental floss increased by 38%. Only 60% of the adults visited a dentist at least once a year. Reduced accessibility of state-run, free-of-charge dental care has caused that over 58% of Poles paid for their dental services. Every 3rd person of working age has not visited a dentist for longer than 2.5 half years, primarily due to behavioural and financial reasons. Oral health behaviours of Polish people are among the poorest in Europe. Despite a noticeable improvement of the behaviours, gap between the Poles and citizens of other highly developed countries is around 20 years. A health promotion programme including oral health issues, if implemented in workplaces, might considerably reduce this gap.
Concomitant contact allergy to formaldehyde and methacrylic monomers in students of dental medicine and dental patients

A multitude of acrylic monomers is used in dentistry. Formaldehyde is a ubiquitous chemical agent, which is an ingredient of some dental materials and may be released from methacrylate-based composites. The purpose of the study is to evaluate the incidence and the risk of cross-sensitization to some methacrylic monomers (methylmethacrylate — MMA, triethyleneglycol dimethacrylate — TEGDMA, ethyleneglycol dimethacrylate — EGDMA, 2,2-bis-[4-(2-hydroxy-3-methacrylo-xypropoxy)phenyl]-propane — Bis-GMA, 2-hydroxy-ethyl methacrylate 2-HEMA, and tetrahydrofurfuryl methacrylate) and formaldehyde in students of dentistry, dental professionals and dental patients. A total of 139 participants were included in the study, i.e., occupationally exposed dental professionals, students of the 3rd, 4th and 6th year of dental medicine, and occupationally unexposed dental patients. They were patch-tested with methacrylic monomers and formaldehyde. The results were subjected to statistical analysis (p < 0.05). From the allergic to formaldehyde students of the 3rd and 4th year of dental medicine, 46.2% were also sensitized to MMA. Among the group of patients, the incidence of cross-sensitization to formaldehyde and methacrylic monomers was as follows: to TEGDMA — 20.6%, to ethyleneglycol dimethacrylate — 20.7%, to 2-HEMA — 20.7% and to tetrahydrofurfuryl methacrylate — 24.1%. Contact allergy to MMA was diagnosed among 22.7%, and to TEGDMA — among 27.1% of the students of the 3rd and 4th year of dental medicine. In the group of occupationally unexposed dental patients the prevalence of contact allergy to ethyleneglycol dimethacrylate was 20.7%, to Bis-GMA — 27.6%, to 2-HEMA — 44.9% and to tetrahydrofurfuryl methacrylate — 38.0%. The students of the 3rd and 4th year of dental medicine could be outlined as a group at risk of sensitization to MMA and TEGDMA and of cross-sensitization to MMA and formaldehyde. Probably, due to the ubiquitous occurrence of formaldehyde and the wide use of composite resins and bonding agents containing TEGDMA, ethyleneglycol dimethacrylate, 2-HEMA and tetrahydrofurfuryl methacrylate in dentistry, the group of dental patients could be at risk of cross-sensitization to formaldehyde and some methacrylic monomers.
A new method of determination of phases and symmetry in stand-to-sit-to-stand movement

Determination of the phases in a stand-to-sit-to-stand task based on a derivative of the parameter with the lowest variance calculated based on the modified symmetry index. The study group comprised 11 healthy women and 11 healthy men from the University of Physical Education in Warsaw. The examination of the kinematic and kinetic parameters of sitting down and standing up from a chair was carried out using the Vicon Mx system and Kistler force platforms. The modified symmetry index was employed to select the parameters used to choose those with the lowest variance. Consequently, the phases in a stand-to-sit-to-stand task were determined. It was demonstrated that the task of sitting down is a reverse task with respect to standing up in terms of the symmetry of the parameters analyzed.
Photobiological safety of the recently introduced energy efficient household lamps

Nowadays more and more newly introduced light sources (tungsten-halogen, compact fluorescent (CFL) and light-emitting diode (LED) lamps) are used in households. It is important to know whether their radiation poses any possible risk for human health or not. These light sources emit optical radiation not solely in the visible spectrum. Other bands emitted by these sources, i.e., ultraviolet and infrared, are potentially hazardous for human health. The visible light, especially the blue light, could also damage human retina. The purpose of this study was to determine the ultraviolet (UV) and blue light (BL) emissions from halogen bulbs, CFLs and LED lamps, and to evaluate them from the point of view of possible health risks for general public. The exposure of UV and BL emissions from 19 types of CFLs, 11 types of halogen lamps and 4 types of LED lamps produced by different manufacturers were measured at 200 mm distance from the source. The exposures to UV and BL were below the International Commission on Non-Ionizing Radiation Protection (ICNIRP) exposure limits in spite of the very conservative assumption of the assessment. Results of this study indicate that the UV and BL radiation from the newly introduced lamps does not exceed the current exposure limit values and thus, in comparison with the former incandescent bulbs, does not result in a higher risk for general public.
An overview of occupational voice disorders in Poland

Occupational voice disorders make the most frequently certified category of occupational diseases in Poland, making up approximately 20% of all cases. This study presents the current knowledge of the etiopathogenesis of occupational voice disorders. It stresses the importance of the evaluation of vocal loading by means of objective measurements. Furthermore, this study discusses the medico-legal aspects of the procedure of certifying occupational voice disorders in Poland. The paper also describes the preventive programs addressed particularly to teachers, including multidisciplinary and holistic management of occupational dysphonia. Their role in the improvement of occupational safety and health (OSH) arrangement for vocally demanding professions is emphasized.
