Lên tới 50% bệnh nhân có tình trạng thiếu kẽm trước khi phẫu thuật giảm béo. Phẫu thuật bypass dạ dày Roux-en-Y (RYGB) là phương pháp phẫu thuật giảm béo phổ biến nhất trên toàn thế giới. Phương pháp này có thể làm trầm trọng thêm tình trạng thiếu kẽm bằng cách giảm lượng thức ăn nạp vào cũng như khả năng hấp thu. Vì vậy, Hiệp hội Béo phì và Phẫu thuật Chuyển hóa Anh khuyến nghị rằng cần theo dõi mức độ kẽm thường xuyên sau khi thực hiện phẫu thuật bypass dạ dày. Tuy nhiên, hướng dẫn của Mỹ không khuyến cáo việc theo dõi này cho tất cả bệnh nhân RYGB mà chỉ dành cho những bệnh nhân có 'những phát hiện cụ thể'. Đánh giá này kết luận rằng tình trạng thiếu kẽm có ý nghĩa lâm sàng là hiếm thấy sau phẫu thuật RYGB. Do đó, việc theo dõi thường xuyên mức độ kẽm là không cần thiết cho những bệnh nhân không có triệu chứng sau phẫu thuật RYGB và nên được dành riêng cho những bệnh nhân có tổn thương da, rụng tóc, ăn bậy, mất cảm giác vị giác, giảm sinh dục hoặc rối loạn cương dương ở nam giới, và thiếu máu do thiếu sắt chưa được giải thích.
