Le cancer colique est en croissance constante dans les pays industrialisés. Le diagnostic se fait trop souvent à un stade avancé de la maladie et est ainsi de mauvais pronostic. La plupart des cancers sont sporadiques et une partie seulement survient chez des sujets à risque. La majorité des cancers se développe à partir d’un polype adénomateux et l’ablation endoscopique de ce polype réduit significativement l’incidence du cancer colo-rectal. Aujourd’hui, la stratégie de prévention du cancer doit viser à identifier les sujets à risque ainsi que les porteurs de polypes sporadiques au sein de la population. Les sujets à risque élevé de développer un cancer peuvent être identifiés par le simple interrogatoire alors que la coloscopie totale est le moyen actuel le plus approprié pour détecter même les petites lésions coliques. Dans le contexte de dépistage de masse du cancer colorectal, la coloscopie totale peut être recommandée à partir de 50 ans pour les sujets sans risque particulier.
