Mặc dù chẩn đoán tâm thần phân liệt, hay còn gọi là sa sút trí tuệ sớm, đã được sử dụng hơn 80 năm, nhưng vẫn còn một sự thiếu rõ ràng lớn về bản chất của "bệnh" này. Một loạt dữ liệu lâm sàng và thực nghiệm ấn tượng đã được tích lũy liên quan đến các đặc điểm của "các bệnh nhân tâm thần phân liệt" nhưng gần như không có dữ liệu nào đặc trưng cho tâm thần phân liệt. Ngược lại, hình ảnh đang nổi lên là một lĩnh vực phức tạp và tương tác của nhiều chiều hoặc yếu tố liên tục. Ba yếu tố cơ bản được gợi ý một cách tuỳ tiện; môi trường nội bộ, môi trường vật lý và môi trường con người. Đề xuất rằng những chiều cơ bản này và các tương tác của chúng theo thời gian nên là đối tượng của sự điều tra và định nghĩa thêm, hy vọng phát triển mô tả đầy đủ và hiệu quả hơn về các hội chứng lâm sàng, thay vì tiếp tục bận tâm với "tâm thần phân liệt" như một nhãn chẩn đoán.
