Ngưng thở khi ngủ tắc nghẽn (OSA) là phổ biến ở bệnh nhân đang điều trị thẩm tách máu, nhưng mối tương quan của nó với bệnh thận mạn tính (CKD) chưa được rõ ràng. Chúng tôi nhằm làm sáng tỏ mối quan hệ giữa OSA không có tăng huyết áp hoặc tiểu đường và tỷ lệ mắc CKD ở Đài Loan. Nghiên cứu theo quần thể này bao gồm các bệnh nhân được chẩn đoán OSA mới từ năm 2000 đến 2009. Nhóm đối chứng được kết hợp theo độ tuổi, giới tính, tiểu đường và tăng huyết áp. Tất cả các đối tượng được chẩn đoán trước đó với bệnh thận cấp tính hoặc mãn tính đều bị loại trừ. Điểm kết thúc chính là CKD mới được chẩn đoán. Chúng tôi đã xác định được 6866 đối tượng bị OSA trong suốt 10 năm nghiên cứu. Thời gian trung bình cho đến khi phát triển CKD trong nhóm OSA là 3,2 năm, sớm hơn 2,5 tháng so với nhóm không bị OSA. Sau khi loại trừ tăng huyết áp và tiểu đường, 4319 bệnh nhân OSA đã được xác định và tỷ lệ rủi ro (HR) của CKD với OSA là 1,37 (95% khoảng tin cậy [CI], 1,05–1,77; p = 0,019). Trong phân tích nhóm, tỷ lệ mắc CKD tăng đã được quan sát thấy ở bệnh nhân nữ bị OSA (HR, 1,41; 95 % CI, 1,12–1,78; p = 0,0036). Nghiên cứu theo dõi quần thể dài hạn này cung cấp bằng chứng rằng bệnh nhân bị OSA ngay cả khi không có tiểu đường hoặc tăng huyết áp có nguy cơ phát triển CKD cao hơn trong 3 năm tiếp theo, và sớm gần 2,5 tháng so với nhóm không bị OSA, đặc biệt là ở nữ giới.
