Ein Längenpolymorphismus in der Promoterregion des Serotonintransportergens (5-HTTLPR) wird sowohl mit der Depression als auch mit der Stressreagibilität in Verbindung gebracht. Gleichzeitig gilt eine Dysregulation des Hypothalamic-pituitary-adrenal(HPA)-Systems als Endophänotypkandidat der Depression. Ziel der vorliegenden Studie war es, einen möglichen Einfluss des 5-HTTLPR-Genotyps auf die HPA-Systemaktivität bei akut depressiv erkrankten Patienten zu untersuchen und Aussagen hinsichtlich einer möglichen Gen-Endophänotypen-Interaktion zu treffen. Es wurden 273 Patienten einer akuten Major-Depression hinsichtlich der 5-HTTLPR genotypisiert und nahmen an einem Dexamethason-corticotropin-releasing-hormone(Dex-CRH)-Test sowie einer Erhebung des Schweregrads psychopathologischer Merkmale mithilfe der Hamilton-Skala (Hamilton Rating Scale for Depression) teil. Patienten mit einem ss-Genotyp zeigten eine signifikant höhere HPA-Systemaktivität als Patienten mit einem ll-Genotyp. Im Hinblick auf den Schweregrad psychopathologischer Merkmale konnte kein derartiger Effekt ermittelt werden. Die Studienergebnisse zeigen einen Einfluss der 5-HTTLPR auf die HPA-Systemaktivität in einer Gruppe depressiv erkrankter Patienten und unterstützen die Hypothese einer Interaktion zwischen den 5-HTTLPR-Genotypen und der HPA-Systemaktivität bei der Pathophysiologie der Depression.
