Cắt bỏ hạch lympho gác là một phương pháp phân loại xuất sắc với độ nhạy trên 95% trong việc phát hiện các hạch lympho dương tính. Khi các hạch lympho gác không có tế bào ung thư, khả năng cao là không có di căn hạch lympho. Các yếu tố hạn chế của phương pháp này bao gồm việc cắt bỏ hormone trong nhiều tháng và tình trạng sau khi cắt bỏ tuyến tiền liệt qua niệu đạo và cắt bỏ tuyến tiền liệt supra pubis. Trong trường hợp đó, kỹ thuật có thể không thành công. Khi các hạch lympho gác dương tính, các hạch lympho không phải hạch gác cũng có thể dương tính. Những di căn hạch lympho này sau đó có thể nằm ngoài khu vực SLN. Do đó, khi có nguy cơ di căn cao hơn, cần kết hợp cắt bỏ hạch lympho gác với cắt bỏ hạch lympho mở rộng.
