Chất lượng cuộc sống và tác động tiêu cực của bệnh đồng mắc ở những người sống sót lâu dài sau ung thư đại trực tràng: So sánh dựa trên dân số

Ung thư đại trực tràng (CRC) là loại ung thư phổ biến thứ ba ở Hoa Kỳ. Mục tiêu của nghiên cứu này là so sánh chất lượng cuộc sống (QoL) giữa những người sống sót lâu dài sau ung thư đại trực tràng và các đối chứng tương ứng không bị ảnh hưởng, đồng thời điều chỉnh theo các bệnh đồng mắc. Để thực hiện nghiên cứu, chúng tôi đã sử dụng cơ sở dữ liệu của Dự án Đăng ký Gia đình Ung thư Đại trực tràng (CCFR) được tài trợ bởi Viện Ung thư Quốc gia (NCI) để ngẫu nhiên chọn lựa và tuyển chọn những người sống sót sau ung thư CRC (≥ 5 năm từ khi chẩn đoán) và các đối chứng phù hợp cho một khảo sát cắt ngang. Chín địa điểm khác nhau về địa lý tại Hoa Kỳ đã tham gia vào nghiên cứu từ CCFR. Các cuộc phỏng vấn qua điện thoại được tiến hành bằng phương pháp hỗ trợ máy tính để đánh giá QoL. Tổng cộng có 403 ca và 401 đối chứng được đưa vào mẫu cuối cùng. So sánh không điều chỉnh cho thấy không có sự khác biệt đáng kể giữa các người sống sót sau ung thư CRC và các đối chứng liên quan đến các chỉ số mệt mỏi, xã hội, cảm xúc, chức năng và thể chất. Phân tích hồi quy logistic đa biến cho thấy tình trạng ca bệnh có ảnh hưởng tiêu cực đáng kể đến các chỉ số QoL đặc hiệu của ung thư đại trực tràng. Chỉ số bệnh đồng mắc cao hơn có ảnh hưởng tiêu cực đáng kể đến QoL tổng thể bất kể tình trạng ca bệnh. Chất lượng cuộc sống của những người sống sót lâu dài sau ung thư CRC tương tự như các đối chứng, với ngoại lệ là các chỉ số QoL đặc hiệu của ung thư CRC kém hơn. Các chỉ số bệnh đồng mắc cao hơn được liên kết độc lập với QoL kém cho cả các ca bệnh và đối chứng. Những người sống sót và các nhà cung cấp chăm sóc sức khỏe nên nhận thức rằng QoL lâu dài tương đương với dân số nói chung; tuy nhiên, có khả năng rằng các vấn đề cụ thể về ống tiêu hóa có thể vẫn tồn tại.
