Prise en charge graduée du syndrome douloureux chronique

La douleur est l’expression symptomatique d’un ressenti où se mêlent des données à la fois somatiques et psychologiques. Lorsque la douleur devient chronique, elle va également s’ancrer hors du patient, imposant durablement une réorganisation de son environnement relationnel. Les relations familiales, professionnelles, sociales et amicales vont être investies différemment par le patient. Ces relations vont être à nouveau modifiées lors du soulagement de la douleur car leur équilibre se nouait autour d’une situation qui tend à s’estomper. Si un nouvel équilibre ne se met pas en place de façon concomitante au soulagement de la douleur, des résistances à la prise en charge apparaissent et/ou l’organisation affective et contingente de l’environnement du patient s’effondre. Cet article se propose d’exposer une approche particulière du soin au patient douloureux, issue de la pratique, tenant compte de cette donnée environnementale et qui vise à ce que le patient trouve progressivement un nouvel équuilibre de vie, en accord avec sa nouvelle situation, moins ou non douloureuse.
