Tóm tắt Giới thiệu Tiêm vaccine phế cầu là một phương pháp phòng ngừa nhằm giảm tỷ lệ tử vong liên quan đến viêm phổi. Tuy nhiên, dữ liệu thực tế về hiệu quả của vaccine phế cầu trong việc giảm tỷ lệ tử vong, đặc biệt ở bệnh nhân cao tuổi, còn thiếu. Nghiên cứu này được thực hiện để đánh giá ảnh hưởng của việc tiêm vaccine phế cầu trước đó ở bệnh nhân viêm phổi cao tuổi. Phương pháp Dữ liệu được thu thập từ cơ sở dữ liệu Đánh giá và Kiểm tra Bảo hiểm Y tế và Đánh giá Chất lượng. Các bệnh nhân nhập viện đáp ứng tiêu chí viêm phổi mắc phải trong cộng đồng (CAP) đã được đưa vào nghiên cứu và được phân nhóm theo tình trạng tiêm vaccine. Bệnh nhân có độ tuổi ≥ 65 và được điều trị bằng beta-lactam, quinolone hoặc macrolide. Bệnh nhân bị loại trừ khi kết quả điều trị không rõ ràng. Kết quả Tổng cộng có 4515 bệnh nhân được đánh giá, trong đó 1609 (35.6%) đã tiêm vaccine trước khi nhập viện. Độ tuổi trung bình là 77.0 [71.0;82.0], 54.2% là nam và chỉ số đồng bệnh Charlson (CCI) trung bình là 3.0. Nhóm bệnh nhân đã tiêm vaccine trẻ hơn so với nhóm không tiêm vaccine (76.0 so với 78.0 tuổi; P < 0.001), và cho thấy sự cải thiện tại bệnh viện cao hơn (97.6 so với 95.0%; P < 0.001) và tỷ lệ tử vong trong 30 ngày thấp hơn (2.6 so với 5.3%; P < 0.001). Sau khi điều chỉnh các yếu tố gây nhiễu như tuổi, giới tính, điểm CURB và điểm CCI, nhóm đã tiêm vaccine cho thấy giảm tỷ lệ tử vong trong 30 ngày một cách đáng kể (tỷ lệ rủi ro [HR] 0.58, khoảng tin cậy 95% [CI] 0.41–0.81; P < 0.01) và tỷ lệ tử vong tại bệnh viện (HR 0.53, 95% CI0.37–0.78; P < 0.001) so với nhóm không tiêm vaccine trong phân tích đa biến. Nhóm đã tiêm vaccine cho thấy tỷ lệ sống sót 30 ngày tốt hơn so với nhóm không tiêm vaccine (kiểm định log-rank < 0.05). Kết luận Trong số các bệnh nhân cao tuổi nhập viện do CAP, việc tiêm vaccine phế cầu trước đây có liên quan đến sự cải thiện tỷ lệ tử vong tại bệnh viện và tỷ lệ tử vong trong 30 ngày.
