Tái phát sau phẫu thuật của bệnh Crohn thường là điều không thể tránh khỏi. Một số yếu tố nguy cơ như hút thuốc, độ tuổi trẻ và hành vi bệnh gây thủng đã được xác định. Những bệnh nhân có hồ sơ nguy cơ cao nên được điều trị bằng mesalamine hoặc azathioprine, trong đó azathioprine có tỷ lệ thành công cao hơn. Đánh giá nội soi của đoạn hồi tràng mới được thực hiện sau phẫu thuật từ 6 đến 12 tháng cung cấp thông tin dự đoán quan trọng về diễn biến lâm sàng của bệnh và có thể được sử dụng như một hướng dẫn để tối ưu hóa liệu pháp y tế.
