Tỷ lệ và các yếu tố liên quan đến mổ lấy thai ở Ethiopia: Phân tích đa cấp từ Khảo sát Nhân khẩu học và Sức khỏe Mini Ethiopia 2019

Tóm tắt

Mổ lấy thai là một can thiệp sản khoa khẩn cấp quan trọng để cứu sống mẹ và trẻ sơ sinh. Tuy nhiên, các yếu tố liên quan đến mổ lấy thai (CS) vẫn chưa được xác định rõ trong dữ liệu cấp quốc gia. Do đó, nghiên cứu này nhằm đánh giá tỷ lệ và các yếu tố liên quan đến mổ lấy thai ở Ethiopia. Dữ liệu từ Khảo sát Nhân khẩu học và Sức khỏe Mini Ethiopia 2019 đã được sử dụng để xác định các yếu tố liên quan đến mổ lấy thai ở Ethiopia. Chúng tôi đã áp dụng hồi quy logistic đa cấp và giá trị p <0,25 để bao gồm các biến trước khi mô hình hóa và giá trị p <0,05 với khoảng tin cậy 95% (CI) cho kết quả cuối cùng. Tỷ lệ mổ lấy thai ở Ethiopia là 5,44% (95% CI; 0,048-0,06) vào năm 2019. Phụ nữ trong độ tuổi 30-39 và 40-49 có xác suất mổ lấy thai cao hơn (AOR = 2,14, 95%CI = 1,55-2,94) và (AOR = 2, 95%CI = 1,20-3,97) so với phụ nữ trong độ tuổi 15-29. Phụ nữ có trình độ học vấn từ trung cấp trở lên có xác suất mổ lấy thai cao hơn (AOR = 2,15, 95%CI = 1,38-3,34) và (AOR = 2,8, 95%CI = 1,73-4,53) so với nhóm không có giáo dục. So với những người theo đạo Chính thống, người Hồi giáo và người theo đạo Tin lành có xác suất mổ lấy thai thấp hơn với AOR là 0,50 (0,34-0,73) và 0,53 (0,34-0,85). Việc có <2 lần sinh cũng liên quan đến tỷ lệ mổ lấy thai thấp với 0,61(0,52-1,22). Sử dụng các phương pháp tránh thai hiện đại, có số lần thăm khám ANC từ 1-3, lần khám thứ 4, 5 trở lên và cư trú ở thành phố có liên quan đến xác suất mổ lấy thai cao hơn lần lượt là 1,4 (1,05-1,80), 2,2 (1,51-3,12), 1,7 (1,12-2,46) và 2,4 (1,65-3,44) 1,6(1,04-2,57). Mặc dù bằng chứng cho thấy mổ lấy thai đã tăng cả ở các quốc gia phát triển và kém phát triển, quy mô hiện tại của dịch vụ này vẫn rất thấp ở Ethiopia, điều này có thể chỉ ra những cơ hội bị bỏ lỡ mà có thể gây nguy hiểm đến tính mạng của mẹ và trẻ sơ sinh. Độ tuổi của phụ nữ, tôn giáo, trình độ giáo dục, số lần sinh, phương pháp tránh thai và lần khám ANC là các yếu tố ở cấp độ cá nhân ảnh hưởng đến mổ lấy thai, trong khi khu vực và nơi cư trú là các yếu tố ở cấp độ cộng đồng ảnh hưởng đến mổ lấy thai ở quốc gia. Dựa trên những yếu tố này, quốc gia cần ra quyết định chính sách cho các can thiệp cấp quốc gia tiếp theo.
