Nghiên cứu thử nghiệm về sự kết hợp giữa S-1 và paclitaxel cho bệnh nhân có di căn phúc mạc từ ung thư dạ dày

Nghiên cứu thử nghiệm này được thực hiện nhằm đánh giá hiệu quả của hóa trị liệu cho bệnh nhân có sự lan tỏa phúc mạc từ ung thư dạ dày hoặc có cytology rửa phổi dương tính được chẩn đoán qua nội soi ổ bụng phân tầng. Mười sáu bệnh nhân đã được ghi danh. Paclitaxel được tiêm truyền với liều 120 mg/m2 vào ngày 1 và S-1 được uống với liều 80 mg/m2 trong 14 ngày liên tiếp, tiếp theo là 1 tuần nghỉ ngơi, như một chu kỳ điều trị. Sau năm chu kỳ điều trị này, các khối u dạ dày chính được đánh giá và phẫu thuật nội soi kiểm tra lại được thực hiện cho những bệnh nhân có phản ứng một phần hoặc bệnh ổn định với lợi ích lâm sàng. Phản ứng một phần hoặc bệnh ổn định với lợi ích lâm sàng được xác nhận ở bảy và năm bệnh nhân, tương ứng, và những bệnh nhân này đã trải qua phẫu thuật nội soi kiểm tra lại. Không phát hiện tế bào ung thư vi sinh khả dụng trong quá trình điều tra bệnh lý tế bào học trong 9 bệnh nhân đã trải qua điều trị phẫu thuật. Tỉ lệ phản ứng theo từng đối tượng điều trị cho khối u dạ dày là 44% và tỷ lệ mất tích của di căn phúc mạc là 38% (6 trường hợp) trong phẫu thuật. Thời gian sống trung bình là 555 ngày. Sự giảm bạch cầu cấp độ 3 và giảm bạch cầu trung tính cấp độ 3 được ghi nhận ở hai và ba bệnh nhân, tương ứng. Chế độ hóa trị này có thể là một lựa chọn chấp nhận được cho bệnh nhân có sự lan tỏa phúc mạc. Chúng tôi dự định sẽ nghiên cứu thêm về chế độ này ở những bệnh nhân ung thư dạ dày có di căn phúc mạc.
