GIỚI THIỆU:

Các bệnh viện ngày càng triển khai các chương trình đa dạng nhằm cải thiện việc nối ghép thuốc và chuyển giao chăm sóc, thường có sự tham gia của dược sĩ.

MỤC TIÊU:

Đánh giá quan điểm của dược sĩ về vai trò của họ trong việc nối ghép thuốc tại bệnh viện và tư vấn xuất viện, đồng thời cung cấp các khuyến nghị của họ để cải thiện chuyển giao chăm sóc.

PHƯƠNG PHÁP:

Mười một dược sĩ tham gia nghiên cứu Can thiệp của dược sĩ cho người có trình độ văn hóa thấp trong bệnh tim mạch (PILL-CVD) tại 2 bệnh viện và thực hiện các cuộc phỏng vấn bán cấu trúc một-một, được mã hóa có hệ thống trong NVivo. Dược sĩ đã cung cấp quan điểm của họ về việc nối ghép thuốc khi nhập viện và xuất viện, tư vấn cho bệnh nhân trong bệnh viện, cung cấp các dụng cụ hỗ trợ tuân thủ thuốc đơn giản (ví dụ: hộp thuốc, lịch trình thuốc hàng ngày có hình ảnh) và gọi điện theo dõi.
