Những điều kiện tổ chức của sản xuất và hiệu quả trong thực hành y tế cơ bản: một nghiên cứu dựa trên dân số

Sự thiếu hụt các bác sĩ đa khoa và sự quan tâm chính trị ngày càng tăng đối với chăm sóc sức khỏe ban đầu đã thúc đẩy sự quan tâm đến phân tích hiệu suất trong lĩnh vực chăm sóc sức khỏe ban đầu ở Đan Mạch. Bài viết này đánh giá mối liên hệ giữa các yếu tố tổ chức của các cơ sở y tế đa khoa và sản xuất cũng như hiệu suất. Chúng tôi giả định rằng sản xuất và hiệu suất có thể được mô hình hóa bằng một hàm sản xuất hành vi. Chúng tôi áp dụng phương pháp ước lượng của Battese và Coelli (Empir Econ 20:325–332, 1995) để thực hiện phân tích phân bổ các biến ngoại sinh nhằm xác định biên sản xuất và các biến xác định khoảng cách của từng bác sĩ đa khoa với biên này. Hai biện pháp khác nhau về đầu ra của thực hành (số cuộc khám tại văn phòng và sản xuất tổng cộng) đã được áp dụng và kết quả được so sánh. Kết quả chỉ ra rằng y tá không thay thế cho bác sĩ đa khoa trong sản xuất. Hàm sản xuất thể hiện tỷ lệ sinh lời không đổi theo quy mô. Mức độ hiệu quả trung bình nằm trong khoảng từ 0,79 đến 0,84, và quy mô danh sách là yếu tố quyết định quan trọng nhất về biến động trong các mức độ hiệu quả.
