Giảm áp bằng ống thông dạ dày trong 1 ngày sau phẫu thuật cắt dạ dày distal: một nghiên cứu ngẫu nhiên tiềm năng

Nhiều bác sĩ phẫu thuật ở Nhật Bản sử dụng phương pháp giảm áp bằng ống thông dạ dày (NGT) trong 1 ngày sau phẫu thuật cắt dạ dày như một quy trình chuẩn. Nghiên cứu ngẫu nhiên tiềm năng này nhằm xác định xem việc giảm áp bằng NGT trong 1 ngày có cần thiết hay không sau phẫu thuật cắt dạ dày distal. Đối tượng nghiên cứu là 233 bệnh nhân bị ung thư dạ dày, được phân ngẫu nhiên thành hai nhóm ngay sau khi phẫu thuật cắt dạ dày distal: một nhóm nhận giảm áp bằng NGT trong 1 ngày (nhóm NGT, n = 119) và nhóm còn lại không thực hiện (nhóm no-NGT, n = 114). Thước đo kết quả chính là các biến chứng liên quan đến phẫu thuật và hô hấp sau phẫu thuật, trong khi các thước đo thứ cấp là quá trình hồi phục sau phẫu thuật và phàn nàn của bệnh nhân. Tỷ lệ biến chứng liên quan đến phẫu thuật không khác biệt đáng kể giữa nhóm NGT và nhóm no-NGT (21,0% và 19,2%, tương ứng; p = 0,87). Tỷ lệ biến chứng hô hấp là 6,7% ở nhóm NGT và 7,0% ở nhóm no-NGT (p > 0,99). Thời gian để thoát khí lần đầu và thời gian nằm viện sau phẫu thuật không khác nhau giữa các nhóm. Hai mươi lăm bệnh nhân trong nhóm NGT và không có bệnh nhân nào trong nhóm no-NGT phàn nàn về khó chịu ở vùng hầu họng (p < 0,0001). Xét về sự khó chịu thể chất do NGT gây ra, chúng tôi tin rằng việc giảm áp bằng NGT trong 1 ngày là không cần thiết sau phẫu thuật cắt dạ dày distal.
