Tóm tắt
Lập kế hoạch chăm sóc trước được khuyến nghị cho những người mắc bệnh thận giai đoạn cuối, nhưng bằng chứng còn hạn chế. Cần có các thử nghiệm lâm sàng vững chắc để điều tra tác động của lập kế hoạch chăm sóc trước trong nhóm dân số này. Có rất ít dữ liệu sẵn có về hiệu quả chi phí để hướng dẫn các nhà ra quyết định trong việc phân bổ nguồn lực cho lập kế hoạch chăm sóc trước. Do đó, chúng tôi đã tiến hành xác định tính khả thi của một thử nghiệm ngẫu nhiên có kiểm soát và kiểm tra các phương pháp để đánh giá hiệu quả chi phí.
Thử nghiệm khả thi có kiểm soát ngẫu nhiên với việc nhập cảnh hoãn lại, kết hợp đánh giá kinh tế và quy trình, với những người mắc bệnh thận giai đoạn cuối, từ 65 tuổi trở lên, đang nhận liệu pháp lọc máu, tại hai đơn vị lọc máu thận ở Bắc Ireland, Vương quốc Anh. Một y tá hỗ trợ đã giúp bệnh nhân lập một kế hoạch chăm sóc trước xác định: người ra quyết định thay thế; những gì người tham gia muốn xảy ra trong tương lai; bất kỳ quyết định trước nào để từ chối điều trị; nơi chăm sóc ưa thích trong giai đoạn cuối đời.
