Chúng tôi trình bày kết quả bệnh lý thần kinh của một người đàn ông 73 tuổi đã qua đời do xuất huyết tiểu não cấp tính trong bối cảnh nhiễm SARS-CoV-2 tương đối nhẹ. Bệnh nhân xuất hiện cơn đau đầu đột ngột, buồn nôn và nôn mửa, ngay lập tức theo sau là mất ý thức vào ngày nhập viện. Dịch vụ y tế khẩn cấp đã phát hiện bệnh nhân bị thiếu oxy nghiêm trọng tại nhà, và bệnh nhân đã trải qua một cơn ngừng tim trong quá trình vận chuyển đến khoa cấp cứu. Đội ngũ cấp cứu đã khôi phục tuần hoàn tự nhiên sau hơn 17 phút hồi sức. Một hình chụp X-quang ngực cho thấy các đám mờ hai bên phổi; và PCR thời gian thực cho SARS-CoV-2 trên mẫu dịch tỵ hầu cho kết quả dương tính. Chụp cắt lớp vi tính đầu cho thấy xuất huyết tiểu não lớn bên phải, với chứng thoát vị amidan và xuất huyết trong não thất. Đáng chú ý, có các nốt vi mô thần kinh và sự tiêu hủy tế bào thần kinh hai bên ở các hạch olive dưới và nhiều vị trí trong các nhân nha tiểu não. Tinh thể dấu hiệu này chưa được báo cáo cho đến nay trong bối cảnh nhiễm SARS-CoV-2.
