La douleur fait partie des symptômes les plus invalidants de la maladie de Parkinson et reste encore mal identifiée et traitée. La stimulation cérébrale profonde des noyaux subthalamiques, ayant déjà nettement démontré son efficacité pour le traitement des symptômes moteurs, permet souvent également de soulager les phénomènes douloureux chez les patients parkinsoniens, via une modulation centrale des seuils de détection et de tolérance de la douleur.
