Methylenetetrahydrofolate reductase (MTHFR), được biểu hiện tại gan, có thể tham gia vào quá trình methyl hóa DNA và tổng hợp DNA. Nó cũng được chỉ định là yếu tố nguy cơ tiềm tàng của ung thư gan ở những bệnh nhân mắc bệnh gan mãn tính. Đến hiện tại, chưa có nghiên cứu nào được thực hiện về MTHFR và ung thư biểu mô tế bào gan (HCC) theo thiết kế dựa trên dân số. Mục tiêu của nghiên cứu này là đánh giá ảnh hưởng của các biến thể đa hình của gen MTHFR đối với nguy cơ ung thư gan nguyên phát và sự thay đổi tiềm năng của chúng đối với các yếu tố nguy cơ môi trường khác nhau. Một nghiên cứu bệnh-chứng dựa trên dân số đã được thực hiện tại Taixing, Trung Quốc. Hai biến thể MTHFR C677T và A1298C được phân tích bằng kỹ thuật PCR-RFLP. Tần số kiểu gen đồng hợp tử 677 C/C của nhóm bệnh là 25,8%, thấp hơn so với nhóm chứng (34,5%). Tỉ số chênh lệch điều chỉnh (OR) cho kiểu gen 677 C/T và T/T của MTHFR lần lượt là 1,66 (95% CI: 1,06–2,61), 1,21 (95% CI: 0,65–2,28) khi so sánh với kiểu gen 677 C/C. Các đối tượng mang bất kỳ kiểu gen T nào đều có nguy cơ tăng 1,55 (95% CI: 1,01–2,40) phát triển ung thư biểu mô tế bào gan nguyên phát. Mức độ liên kết mất cân bằng cao đã được quan sát giữa các biến thể C677T và A1298C, với D′ là 0,887 và p < 0,01. Kiểu gen bất kỳ 677 T của MTHFR được gợi ý có sự tương tác tiềm năng nhiều hơn với việc tiêu thụ nước thô với giá trị p điều chỉnh cho sự tương tác là 0,03. Chúng tôi quan sát thấy rằng kiểu gen 677 C/T của MTHFR có liên quan đến nguy cơ gia tăng ung thư gan nguyên phát trong một quần thể người Trung Quốc. Biến thể đa hình của MTHFR 677 có thể điều chỉnh tác động của việc uống nước thô đối với nguy cơ ung thư biểu mô tế bào gan nguyên phát.
