Béo phì là yếu tố nguy cơ đối với rung nhĩ (AF) và làm thay đổi hiệu quả của điều trị AF xâm lấn. Độ biến dạng theo chiều dài toàn cầu (GLS) của nhĩ trái (LA), được đo bằng siêu âm tim theo dải đốm (STE), là một trong những phương pháp mới giúp đánh giá chức năng của LA. Mục tiêu của nghiên cứu này là đánh giá chức năng LA ở các bệnh nhân béo phì và không béo phì đang trải qua thủ thuật cách ly tĩnh mạch phổi qua da (PVI) trước và 6 tháng sau thủ thuật. 89 bệnh nhân (F/M: 31/58; tuổi trung bình: 55.8 ± 9.8 năm) có AF triệu chứng paroxysmal hoặc dai dẳng đã đủ tiêu chuẩn để thực hiện PVI được đăng ký tham gia nghiên cứu theo phương pháp tiềm năng. Chỉ số khối cơ thể (BMI) được coi là yếu tố phân biệt cho các nhóm nghiên cứu: nhóm béo phì: BMI ≥ 30 kg/m2 (29 bệnh nhân, F/M: 13/16, tuổi trung bình: 55.13 ± 10.1 năm) và nhóm không béo phì BMI < 30 kg/m2 (60 bệnh nhân, F/M: 18/42, tuổi trung bình: 57.17 ± 9.0 năm). Siêu âm tim qua thành ngực (TTE) với GLS LA và biến dạng theo đoạn đã được phân tích 1 ngày trước và 6 tháng sau khi thực hiện PVI. Hiệu quả của PVI được đánh giá 6 tháng sau khi thực hiện PVI qua việc giám sát Holter trong 7 ngày. Phân tích tại thời điểm ban đầu cho thấy LA GLS theo hai buồng (2-Ch) thấp hơn đáng kể ở bệnh nhân béo phì so với nhóm không béo phì (− 10.55 ± 3.7 so với − 13.11 ± 5.1, p = 0.004). Phân tích biến dạng theo đoạn không cho thấy sự khác biệt đáng kể giữa các nhóm. Dữ liệu thu được 6 tháng sau PVI cho thấy LA GLS theo bốn buồng (4-Ch) thấp hơn đáng kể ở bệnh nhân béo phì so với nhóm không béo phì (− 11.04 ± 5.0 so với − 13.91 ± 4.2, p = 0.02), điều này đi kèm với chức năng LA 4-Ch theo đoạn thấp hơn đáng kể ở bệnh nhân béo phì (med-sept: − 11.66 ± 11.2 so với − 15.97 ± 5.3, p = 0.04; api-sept: − 9.04 ± 6.3 so với − 13.62 ± 6.5, p < 0.001; api-lat: − 7.62 ± 4.0 so với − 13.62 ± 6.5, p < 0.001; med-lat: −9.31 ± 7.9 so với − 15.04 ± 6.3, p = 0.003, toàn cầu: − 11.04 ± 5.0 so với − 13.91 ± 4.2, p = 0.02). Hiệu quả của PVI được xác nhận ở 52 (58.4%) bệnh nhân và tương tự ở cả hai nhóm. So sánh giữa biến dạng ban đầu và 6 tháng không cho thấy sự khác biệt nào về LA GLS ở cả hai nhóm. Sự khác biệt về LA GLS trước và sau thủ thuật (delta LA GLS) không phụ thuộc vào tình trạng béo phì. Biến dạng apical-septal và apical-lateral ở nhóm béo phì, được đo bằng cách nhìn theo 4-Ch, thấp hơn đáng kể sau thủ thuật so với mức nền (p < 0.001). Bệnh nhân béo phì có AF paroxysmal được đặc trưng bởi LA GLS bị suy giảm, điều này kéo dài và đi kèm với suy chức năng theo đoạn sau PVI tại thời điểm theo dõi 6 tháng. Hiệu quả của PVI tương đương giữa các bệnh nhân béo phì và không béo phì.
