Bệnh lao gan nguyên phát là một biểu hiện hiếm gặp của lao ngoài phổi ngay cả ở những quốc gia có tỷ lệ nghịch cao. Tỷ lệ mắc bệnh lao gan đã tăng lên trong những năm gần đây do sự gia tăng của HIV/AIDS. Hình ảnh học là một công cụ quan trọng trong việc chẩn đoán, tuy nhiên thường thì các phát hiện hình ảnh không đặc hiệu và có thể bắt chước các bệnh gan lành tính hoặc ác tính khác. Chúng tôi báo cáo một trường hợp của một phụ nữ 54 tuổi được phát hiện có khối u gan trên hình ảnh học nhưng đã bị chẩn đoán nhầm là u nang hydatid. Trong phẫu thuật, các đặc điểm điển hình của u nang hydatid đã không có mặt. Một phần của khối u nang này đã được cắt bỏ. Kiểm tra mô bệnh học của mẫu vật đã cắt cho thấy sự viêm granulomatous bao gồm các tế bào histiocytes và tế bào khổng lồ kiểu Langhans được bao quanh bởi lymphocytes cho thấy kết quả khả nghi về lao gan. Bệnh nhân đã được điều trị bằng thuốc kháng lao trong 1 năm. Hình ảnh tái kiểm tra trong quá trình theo dõi cho thấy sự biến mất của tổn thương gan.
