Gan nhiễm mỡ là một trong những rối loạn gan phổ biến nhất ở các nước phát triển. Đợt dịch bệnh béo phì ở các nước phát triển đã gia tăng cùng với những biến chứng đi kèm, bao gồm hội chứng chuyển hóa và bệnh gan nhiễm mỡ không do rượu. Gan nhiễm mỡ đặc biệt dễ tổn thương do thiếu máu/ tái tưới máu, dẫn đến gia tăng nguy cơ mắc bệnh tật và tử vong sau phẫu thuật gan, bao gồm cả ghép gan. Hiện nay có nhiều hiểu biết hơn về các cơ chế phân tử và tế bào cũng như các phương pháp điều trị cho tổn thương thiếu máu/tái tưới máu ở những bệnh nhân có gan nhiễm mỡ. Bài tổng quan này thảo luận về các cơ chế cơ bản giải thích sự nhạy cảm của gan nhiễm mỡ với tổn thương thiếu máu/tái tưới máu, như rối loạn chức năng ty thể và thay đổi trong dẫn truyền tín hiệu, đồng thời tóm tắt tác động lâm sàng của gan nhiễm mỡ trong bối cảnh cắt gan và ghép gan. Bài viết này cũng mô tả các phương pháp điều trị tiềm năng, như tiền điều kiện thiếu máu và điều trị bằng dược phẩm, để ngăn ngừa tổn thương thiếu máu/tái tưới máu ở những bệnh nhân có gan nhiễm mỡ. Các phương pháp khác, bao gồm tưới máu bằng máy móc, cũng đang được nghiên cứu lâm sàng; tuy nhiên, nhiều phương pháp dược lý được phát triển thông qua nghiên cứu cơ bản vẫn chưa phù hợp cho ứng dụng lâm sàng.
