Một chương trình đào tạo có cấu trúc là cần thiết cho việc áp dụng an toàn phẫu thuật trực tràng bằng robot. Mục tiêu của nghiên cứu này là mô tả con đường đào tạo và kết quả phẫu thuật ngắn hạn của ba bác sĩ phẫu thuật tại hai trung tâm (Anh và Bồ Đào Nha) thực hiện phẫu thuật trực tràng bằng robot với hệ thống da Vinci Xi và chuyển động bàn tích hợp (ITM). Dữ liệu được thu thập theo cách triển vọng cho các bệnh nhân liên tiếp trải qua phẫu thuật cắt bỏ ung thư trực tràng bằng robot với hệ thống da Vinci Xi và ITM trong khoảng thời gian từ tháng 11 năm 2015 đến tháng 9 năm 2017 đã được phân tích. Kết quả phẫu thuật ngắn hạn của mười ca đầu tiên của mỗi bác sĩ (được giám sát) đã được so sánh với các ca sau đó (tự lập). Ngoài ra, các biểu mẫu Điểm đánh giá toàn cầu (GAS) từ các ca được giám sát cũng đã được phân tích và biểu đồ CUSUM (cumulative sum) của GAS được xây dựng để điều tra con đường đào tạo của các bác sĩ phẫu thuật tham gia. Dữ liệu từ 82 bệnh nhân đã được phân tích. Không có ca chuyển sang phẫu thuật mở, không có rò rỉ anastomotic và không có tử vong trong 30 ngày. Thời gian phẫu thuật trung bình là 288 phút (SD 63), lượng máu mất ước tính trung vị 20 (IQR 20–20) ml và thời gian lưu viện trung vị là 5 (IQR 4–8) ngày. Tỷ lệ tái nhập viện và phẫu thuật lại trong 30 ngày lần lượt là 4% (n = 3) và 6% (n = 5). Khi so sánh các ca phẫu thuật được giám sát với các ca phẫu thuật tự lập sau đó, không có sự thay đổi có ý nghĩa thống kê nào trong bất kỳ kết quả ngắn hạn nào ngoại trừ thời gian phẫu thuật trung bình, thời gian này ngắn hơn một cách đáng kể trong các ca tự lập (311 so với 275 phút, p = 0.038). Phân tích biểu mẫu GAS và lập biểu đồ GAS CUSUM cho thấy rằng mười ca giám sát là đủ để các bác sĩ phẫu thuật trong quá trình đào tạo thực hiện độc lập các ca cắt trực tràng bằng robot với hệ thống da Vinci Xi. Kết quả của chúng tôi cho thấy rằng bằng cách áp dụng một con đường đào tạo có cấu trúc và chuẩn hóa kỹ thuật phẫu thuật, quy trình đơn dock với hệ thống da Vinci Xi là một kỹ thuật hợp lệ, có thể tái sử dụng và mang lại kết quả ngắn hạn tốt trong quần thể nghiên cứu của chúng tôi.
