Het verband tussen therapeutische interventies en productieve emotionele verwerking bij de Attachment-Based Family Therapy (ABFT) voor depressieve en suïcidale jongeren

Productieve emotionele verwerking (productive emotional processing) wordt beschouwd als een cruciaal verandermechanisme binnen de attachment-based family therapy (ABFT). In dit artikel bestuderen we het effect van ABFT-interventies die gericht zijn op productieve emotionele verwerking van primaire adaptieve emoties. De studie heeft betrekking op een steekproef van dertig depressieve of suïcidale jongeren die deelnamen aan een groter gerandomiseerd klinisch onderzoek. Uit de resultaten van sequentiële analyses blijkt dat het op gang brengen van de productieve emotionele verwerking van primaire adaptieve emoties bij de adolescent samenhangt met direct daaraan voorafgaande reframing of specifieke focus van de therapeut op primaire adaptieve emoties. Andere benaderingen van de therapeut – zoals interpretatie, geruststelling, of focus op afwijzende kwaadheid van jongeren jegens hun ouders – blijken een tegenovergestelde werking te hebben, in de zin dat die leiden tot onderbreking van de emotionele verwerking als die al begonnen is. En ten slotte is ook gevonden dat, als de therapeut zich richt op aanmoediging van affect in het algemeen, of op onvervulde gehechtheidsbehoeften van de adolescent, het effect beide kanten op kan gaan. Dat wil zeggen dat deze interventies zowel kunnen leiden tot initiëring van productieve emotionele verwerking als tot een onderbreking van dat proces als het al gaande is. In dit artikel worden de theoretische en klinische implicaties hiervan besproken.
